Rur al Health Service Corps will have full- and part-time positions starting in Fall 2019! AmeriCorps is a network of national service programs, made up of three primary programs that each take a different approach to improving lives and fostering civic engagement.

Members commit a year of their time (or less) to address critical community needs like increasing  academic achievement, mentoring youth, fighting poverty, s ustaining national parks, preparing  for disasters, and more. 

Positions with RHSC involve combating the opioid epidemic, increasing health access and education, and improving food security. A full listing of our positions can be found here. capitalCapital Campaign Update
                                 
Last month, Rural Health Network announced the kick-off of a public appeal for support of a Capital Campaign to purchase and renovate our headquarters at 455 Court Street in Binghamton. 

We have already reached 40% of the goal to raise a total of $525,000. Initial support has been committed by the Dr. G. Clifford and Florence B. Decker Foundation, George A. and Margaret Mee Charitable Foundation, United Way of Broome County, Inc., and Rural Health Network board and staff members. Food Food and Health Network Logo

The Downtown Binghamton Courtyard Market officially started last month! Relaunching the market was a combined effort of Rural Health Network, VINES, the City of Binghamton, multiple Binghamton businesses, and several community members. It will be held every Thursday, until the end of October from 3:30-6:30 at the Metrocenter Courtyard in Downtown Binghamton. The market is a space for community members to shop for goods grown or produced in a 50 mile radius by craftsman, farmers, and businesses. Maggie Caroompas, Farm to School AmeriCorps member with the Food and Health Network, joined the Kids' Farmers Market last month to sample a healthy snack.  The market is hosted every Thursday from 12-1 at Common Ground Christian Community Center in Owego. The Food Bank of the Southern Tier supplies the markets with produce such as green beans, zucchini, apples, plums, and more for children and their parents who stop in over the summer. Families enjoyed a fruit salad with a creamy dressing while picking out their free fruits and vegetables.

Rose Peguero, Community Health Worker I

Welcome to our new Community Health Worker, Rose Peguero.  She is assigned to Broome County Medicaid Members accessing new primary care providers and Tioga clients managing their diabetes or chronic respiratory health issues.  

Rose Peguero has just returned to the Southern Tier after successful assignments in both upstate and downstate regions of New York as a Parent Educator, as a Sr. Employment Specialist doing recruiting, and in Credentialing. Rose worked as the Assistant Manager of Recruiting for the US Census Bureau in the 2010 Decennial and was responsible for nine counties which included rural, urban, and suburban. Rose is very proud of her work as an allied health instructor. She  is a graduate of SUNY- Empire College where she received her Bachelor's degree in Human Services with a concentration in Healthcare Advocacy and completed work in not for profit case management. Rose also has degrees in Medical Assisting and Business Administration and attended a Bilingual Business Administration in the Dominican Republic. She looks forward to connecting people to our clinical partners in health care from Gerould's , Guthrie, Lourdes and UHS and to  pursuing her passion for rehabbing furniture now that she is back in the Southern Tier.

Justina Lewis and Celena McDonnell, Mobility & Transportation Advocates

Two new Mobility and Transportation Advocates have joined the Getthere team. 

Celena McDonnell spent many years working as a nursing assistant and customer service professional. She became interested in our program during her volunteer work on the Binghamton-Broome Anti-Poverty Initiative, where she learned about the work the Rural Health Network was doing to connect clients in need with transportation. Knowing there was such a large part of our area's population that were missing out on the opportunity to care for their health due to inadequate transportation options, Celena jumped at the chance to transform her work experience and love of volunteering into a new career.  In addition to her Mobility and Transportation Advocate work, she enjoys spending her time writing non-fiction and poetry and with her wonderful children.

Justina Lewis has worked within the healthcare field for the last 11 years. She has worked as a CNA in skilled nursing facilities and also did home health for two years. She graduated from SUNY Broome in 2012 with a certificate in phlebotomy. Justina has always wanted to pursue a career in which she would be helping people.  After hearing about the Getthere program she knew that this was something that she could be passionate about. Justina said: "Growing up in a rural area gives a whole new understanding to what this program means. For so many people this program is an amazing opportunity to be able to stay more independent."  In her free time Justina enjoys spending time with her husband and 4 children. She is an avid reader and has an extensive Stephen King collection.

SdohUpdate on the Social Determinants 
Measurement Project: CBO Dashboard
In the shift toward Value Based Payment (VBP) models, New York State encourages the inclusion of community-based organizations (CBOs) in clinical-nonclinical partnerships and multi-sector networks to address social determinants of health (SDOH). While the healthcare sector may be accustomed to organizing among clinical and insurance providers, the process of bringing in CBOs is less familiar. One significant concern is that potential VBP partners are not aware of CBOs in their region, or have limited knowledge of CBO preparedness and willingness to enter VBP contracts.

To address this issue, the RHN Social Determinants Project team developed a template for a searchable "dashboard" of CBOs, intended for use by potential VBP partners - healthcare providers, payers, etc. The dashboard would provide information on CBOs' social determinants programs, geographic footprint, client numbers, outcomes, willingness to expand, and other measures of interest to potential partners. The template developed by the project team will inform Care Compass Network PPS's ongoing effort to build a regional CBO listing with additional functions. Similar efforts are underway at the state level through the NYS Bureau of Social Determinants.

Bill Wagner, Director of Getthere Mobility Management Programs completed  the CTAA, Community Transportation
Association of America, 2018-2019 Emerging Leaders Academy and presented on Getthere's Transportation to Employment Project at the CTAA Expo.

Chronic Disease Self Management Program
                                     
The Chronic Disease Self-Management Program is a workshop for anyone who has or is at risk for a long-term health condition, OR a caregiver of a person with a long-term health condition. The groups meet once per week for 6 weeks, and focus on action planning, problem solving, communicating with health-care providers, and other skills related to managing our own health.

Rural Health Network will be holding workshop series in Broome, Tioga, and Delaware Counties this fall. For more information, contact the Director of Community Health Education, Mary Maruscak.

All of Us Research Program Seeking More Participants

The All of Us Research Program invites people ages 18 and older, regardless of health status, to join this momentous effort to advance individualized prevention, treatment and care for people of all backgrounds. Part of the National Institutes of Health, All of Us is expected to be the largest and most diverse longitudinal health research program ever developed. The program is seeking one million or more people from all walks of life to participate in this historic endeavor.
