  1. Missing Alta 9 or cleared survey
  2. Missing MIN Summary for 1st Note
  3. Missing stamped “Certified True Copy” of the DPA Mortgage/Deed of Trust or evidence that it has been recorded
  4. Missing evidence that the FHA UPMIP has been paid
  5. Missing copy of the MHDC Approval Form 195
