• Home Delivery copays have changed. We always recommend that members use our Home Delivery mail order program prescription maintenance medications. The copays for these prescriptions have increased for a 90-day supply for generic formulary drugs from $7 to $10; for brand-name formulary drugs from $24 to $29; and for non-formulary drugs without a medical necessity prior-authorization from $53 to $60.

• PRP injections are now covered for mild to moderate osteoarthritis of the knee and for lateral epicondylitis. This will require prior authorization.

• 3-D mammography is now covered for breast cancer screening.

• Members who are trying to quit smoking can receive nicotine replacement products with a prescription through the Home Delivery program. These products are not available from our network retail pharmacies.

• Breast pumps and supplies and breastfeeding counseling are covered, but there are some new restrictions to the maximum amount allowed.
