Pike County diabetes coalition is a member of a five-county collaborative call the Big Sandy Diabetes Coalition (BSDC). The coalition, based at Big Sandy Health Care in Prestonsburg, aims to improve detection, prevention and treatment of diabetes through screening and connection with local resources; it serves the five southeastern counties of Floyd, Johnson Magoffin, Martin and Pike.

Community Diabetes Screenings:
During a community screening, a man had a very high A1C. As a result of the educational information he received about the benefits of proper nutrition and physical activity, he walked his first 5K less than two weeks after the initial screenings.

:
Community exercise programs are implemented at several sites including senior centers and faith organizations. Coalition members have implemented the program since 2015. Gentle Yoga For All (Chair Exercise) provides an enjoyable fitness program for anyone whose physical activity is limited due to age, disability, recovery from substance abuse, or any chronic conditions. The emphasis of the program is proper breathing, stretching and relaxation techniques. Gentle Yoga is a coalition main stay and participant favorite.

June is National Men's Health Month!
|
|
Fear of receiving bad news is the number one reason men don’t talk about or take better care of their health. Historically, men have not been comfortable discussing issues about their health, particularly conditions like diabetes and depression. This has resulted in shorter and less healthy lives for men in the United States compared to women. We encourage all men with diabetes to take the "modern man" challenge: get out, get active, get informed!

The Comprehensive Resource for Achieving Success in Diabetes Self-Management Education Support

|
Diabetes self-management education and support (DSMES) has been shown to improve health outcomes. The purpose of this toolkit is to increase use of DSMES services among people with diabetes and promote healthcare provider referrals. Expanded use of DSMES can help ensure that all people with diabetes receive the support they need. The toolkit provides resources and tools in one place to assist with the development, promotion, implementation, and sustainability of DSMES services.

The purpose of the Farmers Market Promotion Program (FMPP) is to increase domestic consumption of, and access to, locally and regionally produced agricultural products, and to develop new market opportunities for farm and ranch operations serving local markets by developing, improving, expanding, and providing outreach, training, and technical assistance to, or assisting in the development, improvement, and expansion of, domestic farmers markets, roadside stands, community-supported agriculture programs, agritourism activities, and other direct producer-to-consumer market opportunities.

Interactive Reporting Tool to Assist Appalachian Communities in Exploring Community Data
Are you looking for health statistics for your community, county, or state? Creating a Culture of Health in Appalachia: Disparities and Bright Spots is an interactive report that includes county-level data on 41 variables, including several measures relevant to diabetes and its risk factors. Users can easily create reports or maps to examine conditions in any county, or compare multiple counties. Creating a Culture of Health in Appalachia: Disparities and Bright Spots is an innovative research initiative sponsored by the Robert Wood Johnson Foundation (RWJF) and the Appalachian Regional Commission (ARC) and administered by the Foundation for a Healthy Kentucky.

The purpose of the National DPP Customer Service Center is to provide organizations easy access to information and resources about prediabetes and the National DPP. Organizations can access training materials, toolkits, and videos; ask questions; and receive technical assistance related to all aspects of the program.
