Two Upcoming Funding Opportunities for All In

This month, DASH will release two new funding opportunities for All In members: 1) a new round of CIC-START awards, which provide support to local collaborations for activities that build capacity to share data and 2) funding to participate as peer-to-peer mentees in the new DASH Mentor Program. Both programs will launch by the end of the month, so watch your inbox for details and webinar registration information. 

Measuring Social Needs and Outcomes: Key Considerations

How can health care organizations measure social needs in an actionable way? This is a question that many communities are grappling with as they strive towards coordinated whole-person care that addresses social determinants of health. Presenters on a recent  All In webinar highlighted key considerations for measuring social needs in health care settings.

NNIP Resources on Data Governance, Security, and Technology for Civic Data Integration

The National Neighborhood Indicators Project (NNIP) released two new resources: 1) a guide to help organizations develop robust data governance and security programs and 2) a report offering technical guidance for jurisdictions linking administrative data across agencies.


New: Neighborhood Life Expectancy Data

The United States Small-Area Life Expectancy Estimate Project (USALEEP) released new census-tract level data on life expectancy. This is the first time life expectancy at birth estimates are available nationwide down to the neighborhood level.


Community Care Coordination Systems: Technology Supports 

DASH Co-Director Clare Tanner was one of several experts interviewed for this issue brief from Nemours, which explores elements of technology applications emerging to "close the loop" between clinical and community services.

Care AC, a NJHI awardee, is partnering with transit police in Atlantic City on a program that offers residents a chance to get their warrants recalled temporarily and, in the process, access the services they may need to get themselves back on their feet.


Case Study: Identifying Child Lead Poisoning Risk

AllianceChicago, a former DASH awardee partner, published a case study describing their use of a clinical decision support tool that leverages multi-sector data to help identify young children at risk of lead exposure.


New Report Identifies NYC Neighborhoods Where Medicaid Might Better Support Members' Health by Addressing Housing Insecurity

United Hospital Fund recently published a brief on how Medicaid might address housing and health in NYC using neighborhood tabulation area data collected by a former DASH awardee to map housing instability, emergency department use, and Medicaid enrollment.

Imperial County Accountable Communities for Health

With support from CACHI, multi-sector partners in Imperial County, CA are developing a comprehensive effort to reduce asthma hospitalization using the Accountable Communities for Health model. 


Bridging Health and Safety in Near Northside

This BUILD partnership in Houston, TX is taking steps to address several upstream health and safety issues, including improving infrastructure, reducing crime, implementing food-as-health and exercise-as-medicine programs, and ensuring healthy, affordable homes.

  • PRAPARE Innovation and Engagement Awards: Due October 26 - This award recognizes innovative uses of the PRAPARE social determinants of health assessment tool to disseminate those models and lessons learned.
