FH Awareness Week is a time to remember why we are all working together to find individuals with familial hypercholesterolemia (FH) and improve their diagnosis and care.

Over five years ago, the FH Foundation set out to change the status quo with an innovative, patient-centered, data-driven approach. Together, we have made progress, but there is so much more to do.
FH Awareness Day - est. 2012
 
Despite affecting 1 in 250 individuals, FH remains largely invisible, which is why on Sept. 24 we shout from all the rooftops - through Twitter,  articles,  billboards, and radio and TV interviews. Add your voice to the #KnowFH hashtag and by speaking with your friends, family and community.

FIND FH ®

The FIND FH initiate aims to identify the 90% of the undiagnosed FH population in the U.S. Using novel informatics and big data including over 200 million healthcare records, FIND FH "flags" individuals with profiles consistent with FH for follow up care.
CASCADE FH Registry
 
The CASCADE FH Registry collects longitudinal data  through a Patient Portal and from 40 sites across the U.S.  Real-world data from the registry highlights the difference between "what we do" and "what we should be doing." It drives our understanding of existing gaps in diagnosis and treatment and helps us model future best practices.
FH Global Summit
 
The annual FH Global Summit will gather experts from around the world to improve the understanding, diagnosis and treatment of FH and highlight FH as a prime target for precision public health. This year, we focus on diverse subpopulations including women, children and ethnic groups, as they are significantly less well managed and a higher risk for poor outcomes. 
FH Advocates on Capitol Hill
 
Data drives policy and policy drives action. FH Foundation Advocates from across the U.S. visited over 20 congressional offices to share messages to drive change through the message that we need to do more to address FH as a publilc health priority.
FOCUS
 
FOCUS was launched to take a data-driven approach to understand access barriers to care and therapies for those with FH. We educate all stakeholders - from physicians to patients and payers - about opportunities to optimize FH care and advocate for change.
