Jump Start – Kindergarten – First Grade

EVERYTHING YOU NEED TO KNOW ABOUT JUNE:

Friday 6/1 – TODAY is the Deadline to order a MAP Video Yearbook – it’s your last chance to own all of these amazing memories collected throughout the year!

Thursday 6/14 – Last Day of Jump Start MAP (due to transition day 6/15 & the last day of school being an early release day) When Jump Start tuition was calculated, these days were not included.   If you have any questions, please contact Gaye gayeshannon@verizon.net

Friday 6/18 – Transition Day at Memorial School . (No Jump Start MAP) All Ks go to the AM Session, and if it is their normal MAP day they will be released to MAP at 11:40 and may stay until 2:30 or their usual pick up time. All Ks need to come to MAP with a lunch. First grade will be released at their usual time. If your child will not be coming to sure to let MAP and their teacher know.

Monday, 6/18 – Last Day of School– Early Release Day
·          No Jump Start MAP
·          There is NO PM Kindergarten (No AM MAP- required special sign up)
·          Children in AM K, Full Day K & First Grade will be released to MAP at 11:40. 
·          All children must come to MAP with a lunch. Please remember to call MAP at (508) 359-2165 if your child will not be attending!

Tuesday 6/19 - Thursday 6/21 – MAP Special Sign Up Days – MAP will be open from 8-6 for families who have signed up (not available for Jump Start MAP).

Thursday 6/21 - Come CELEBRATE the end of the school year, the last day of MAP, and KICK OFF YOUR SUMMER at the MEMO Concert at the Gazebo next to the Medfield Library. MAP will be sponsoring this event and it’s sure to be a BALL! Hope to see you there!

Friday 6/22 – MAP is CLOSED – We hope everyone has a safe and wonderful summer!
 
On your child’s last day of MAP please be sure to….
Collect any medication that MAP has for your child.
Pick up any spare clothes or sunscreen.
Check the lost and found, as any items left will be donated.
YOUR SUMMER TO DO LIST:

2018-2019 Child Information Forms DUE back to MAP by August 10, 2018 -
Forms will be available on our web page and an email reminder will go out at the end of JUNE. ALL MAP CHILDREN must have a NEW CHILD INFORMATION form completed each year. Jump Start Families must also submit a copy of their child’s most recent physical & a developmental history.
 
Allergies, Asthma, or a Chronic Health Condition? Families must complete:
·          Severe Allergy Action Plan (antihistamine, EPI Pen)
·          Individual Health Care Plan (for any chronic condition, other than severe allergies, including but not limited to asthma, ADD/ADHD, diabetes, cancer, epilepsy, and/or non-severe allergies
·          Medication Consent form (for both prescription and non-prescription medications that are NOT necessary for a severe allergy or chronic condition (ibuprofen, antibiotics, etc.). 

*Please note ALL FORMS REQUIRE A DOCTOR or HEALTH CARE PROVIDERS SIGNATURE. Forms can be found @ www.medfieldafterschoolprogram.com & MAP’s Health Care Policy is in our Family Handbook.  


Planning Ahead for 2018-2019….
  •  Friday, August 10 - CHILD INFORMATION FORMS DUE – These forms will be emailed to you in late June/early July and will also available on our website. These forms are not the same as the registration form, they have much more detail and MAP must have these forms completed before your child can start MAP. 
  • Friday, August 10 - If your child has an allergy, chronic medical condition, or a special health or medical issue, there is additional paperwork that must complete prior to their attendance. These forms are available on our web site www.medfieldafterschoolprogram.com Please note that all of these forms require a Doctor’s Signature. MAP also requires all families to call or email us to set up an appointment to meet and drop off all necessary medications and required paperwork. MAP must have all completed forms & medications by Friday, August 10.
  • Thursday, August 23 - Save the date for our OPEN HOUSE to see the program and meet the teachers as well as other children and families who will be at MAP.   Jump Start & Half-Day Kindergarten 4:30-5:15 Full Day Kindergarten 5:30-6:15 First Grade 6:30-7:15
  • Tuesday, August 28– MAP will be offering a Special Sign up for children in Kindergarten- 6th Grade. MAP will be open 8 a.m. until 6 p.m.
  • Wednesday, August 29 – First Day of School for children in grades 1 - 12
  • Tuesday- August 28–Friday, August 31: MAP will be offering a Special Sign up for children in Kindergarten from 8 a.m. until 6 p.m. *not available for Jump Start.
  • Friday, 8/31 – Early release day
  • Monday, 9/3 – No School – MAP Closed 
  • Tuesday, 9/4 – First Day Jump Start & Kindergarten
  • Monday, 9/10 & Wednesday 9/19 – NO SCHOOL – MAP will be offering a Special Sign up for children in Kindergarten- 6th Grade. MAP will be open 8 am – 6 pm. *not available for Jump Start
  • Saturday, September 15 - Medfield Day! MAP will have a booth and we will offer opportunities to help via an email in early September….
The MAP Calendar for 2018-2019 will be going out soon!
 
While MAP is closed for the summer, Gaye Shannon, the Executive Director, and the Program Directors check their email and phone messages periodically. Please don’t hesitate to reach out to us if you have any questions or concerns. We will do our best to get back to you as promptly as possible.

     Thank you for sending your children to MAP to learn, laugh, play, make friends, join clubs, try new things, play games, be active in the gym, be artistic & crafty, and grow from all that MAP has to offer! Good luck to the soon to be 2nd graders going to 2-3 MAP in the fall! We look forward to seeing the new Jump Start, Kindergarten and 1st graders back here at MAP! We hope you have a great summer!
