NAMI Survey - Children's Behavioral Health Service System

NAMI is conducting a statewide needs assessment focused on the children's behavioral health service system within our Medicaid program, also known as AHCCCS (Arizona Health Care Cost Containment System).

The purpose of the needs assessment is to:

1. determine the current climate (since 10/1/2018) for Arizona's children and families
2. identify barriers that need to be addressed within the children's system.

By completing this survey, you will help towards research of understanding families' resources and needs.

Please complete this survey IF:
1.        You are a provider within the AHCCCS children's behavioral health system
2.        You are a AHCCCS service recipient and have obtained services within the children's behavioral health service system

This will take approximately 3-5 minutes of your time. There are no risks to you for participating, and no penalty for not participating.

The survey will close on June 30th, 2019.

Taking a mental health screening is one of the quickest and easiest ways to determine whether you are experiencing symptoms of a mental health condition.
Mental health conditions, such as depression or anxiety, are real, common and treatable. And recovery is possible.
