Bridge to Health Careers Training Program

Make the Road New York's Bridge to Health Careers Training Program
 
Make the Road New York's free Bridge to Health Careers course is designed for high level English language learners who are interested in exploring healthcare career options. Students will participate in depth reading, writing, listening and speaking activities designed to foment language skills both in and out of class. This course will focus on academic and professional English and build proficiency around important skill areas through research projects, presentations, group activities as well as essay, resume and cover letter writing. Students will also learn about the various dimensions of health and explore health issues that affect these dimensions and their communities.  

The class will examine how to best change negative health behaviors in potential clients, communities and in themselves. After reflecting on their own skills, traits and experiences, students will explore the field of health care to determine which career they are best suited for. The class will examine how to best achieve their healthcare career goals and what next steps are necessary. Finally, students will apply to additional colleges as well as certification or training programs to make their healthcare career goals become a reality. (PLEASE NOTE: This course is designed to help students gain access to college and/or other healthcare certification programs. Bridge Certification will not lead directly to employment opportunities.)

Participants in this program will:
  • Improve their academic and professional English in order to take meet the demands of the healthcare labor market.
  • Learn important health care and career work-related vocabulary and concepts
  • Receive career-readiness training to pursue their career goals and determine next steps
  • Receive assistance in applying to colleges as well as certification and training programs.
  • Receive support accessing follow-up training programs if/as appropriate

Make the Road NY's Community Health Worker Training Program
 
Community Health Workers (CHWs) help members of the community stay healthy by helping them navigate the healthcare system, obtain access to healthcare, provide health education and referrals to other services. Make the Road New York's free Community Health Worker Training Program (CHW) is for highly-dedicated bilingual students with Advanced English language skills. This course will provide participants with the health-related, and job skills needed to begin and build careers as Community Health Workers. 

Participants in this program will:
  • Learn Community Health Worker skills such as outreach, informal counseling, health education and will gain an understanding of public health and the health care system in the U.S.
  • Learn how to use technology in community health work
  • Complete 135 internship in a hospital, clinic or community-based organization
  • Receive healthcare career counseling and placement support
  • Get a basic orientation in a variety of health topics through our health specialties and Health and Safety trainings

Patient Activation Training
 
Patient activation refers to how capable an individual is of taking care of themselves when it comes to managing their health and healthcare. Different people have different levels of activation. In this training, participants will learn how to assess a person's level of activation using the Patient Activation Measure (PAM©) and how to tailor goals and interventions for an individual based on their level of activation using Coaching for Activation (CfA©). Be sure to register using the link below. Care Coordination Basic and Advanced

Care Coordination Basic and Advanced

The Care Coordination Fundamentals curriculum consists of four (4) modules or sections that are structured to build on one another sequentially. All front-line staff working in a team-based healthcare environment will benefit from this course. This advanced version of the care coordination curriculum introduces staff to chronic, mental and behavioral conditions, (i.e. diabetes, hypertension, high cholesterol, asthma, depression, substance abuse etc.) and helps them understand these conditions and provides participants with coaching tools on how to teach patients to manage their health.

Participants will experience a highly interactive class environment tailored to adult learners. Our approach strengthens the participants' critical thinking skills by engaging them in discussion, individual exercises and group activities. Participants complete the course prepared to assist their patients in navigating the healthcare system, and to be strong, productive members of healthcare teams providing coordinated and patient-centered care.  Be sure to register using the link below. Trauma Informed Care

Trauma Informed Care

Psychological trauma is highly correlated with chronic health conditions, substance use disorders and mental health problems. This course will help health care providers understand the nature of psychological trauma and it's biological, psychological, social and behavioral impacts. The signs and symptoms of post-traumatic stress disorder and complex trauma will be reviewed and participants will learn how to screen for trauma exposure and PTSD symptoms. Participants will learn how trauma can be addressed in treatment, both in specific treatment approaches and in milieu practices that are conducive to keeping patients engaged in care. The premise of this course is that day-to-day experiences and interactions between traumatized patients and all staff can have a bearing on their ability to remain in treatment and make clinical progress. Health Literacy for Public Health Professionals

Health Literacy for Public Health Professionals

Offered by the Center for Disease and Control, the goal of Health Literacy for Public Health Professionals is to introduce participants to the fundamentals of Health Literacy and demonstrate the importance of it within public health practice.

This course consists of a brief introduction followed by three lessons. Within these lessons, there are video clips, case studies, and knowledge check questions.

The Depression CAREPATH was developed for use in home health care and hospice to support the assessment, monitoring, and case coordination of depression in older adults. Patient education materials, communication aids, and implementation guides and resources for administrators are also available.  After you complete registration for HWapps, the link found in the content section will direct you to a registration form for mental health training network, where you will access training.

LGBTQ Cultural Awareness
 
LGBTQ Cultural Awareness

LGBTQ Cultural Awareness Training, presented by the Pride Center of Staten Island, advances a person-centered approach to working with LGBTQ patients and/or clients in healthcare settings. The training includes an introduction to (or review of) LGBTQ terminology, activities that focus on intersectionality and structural systems of oppression, and a discussion of the health disparities confronting LGBTQ individuals. Additionally, the training will give you concrete tools to make healthcare environments more welcoming for LGBTQ patients and/or clients, as well as your LGBTQ co-workers.

Person Centered Care Services of Staten Island is striving to develop relationships with agencies, businesses, hospitals and all community members to provide training/education on interacting/working with people with disabilities. After people are given the education of disabilities culture, communication tips, and general etiquette, better relationships are possible. This training can be geared and modified for anyone in any role or capacity. For example, if they are training hospital staff, the training can be geared to focus on health literacy and communication/comprehension of medical issues/treatment.

Health Literacy  & Teach Back

This module is part of Care Management Training. Understanding the barriers that health literacy can present for patients is a key component of care. This module provides the learner with the many aspects of health literacy that need to be considered when educating a patient. In addition, the information points out key indicators that patients may be struggling to understand and also discuss how to employ teach back and other methodologies to be sure patients understand.
