CHRONIC DISEASE SELF-MANAGEMENT PROGRAM

Community Engagement Network and the Long Island Health Collaborative are offering the Chronic Disease Self-Management Program (CDSMP), a six-week interactive workshop that provides hands-on interactive and fun education for people and their families or care partners who are living with chronic conditions.  To make arrangements to host at your location, contact Janice Hidalgo at 917-648-6528.
JOIN SCC FOR CULTURAL COMPETENCY AND HEALTH LITERACY MASTER TRAINING

The communities we serve are growing in cultural and linguistic diversity. There is a need to inspire cultural change within organizations.

On Tuesday, December 3, 2019, 9:00 am to 4:00 pm, SCC will be holding Cultural Competency and Health Literacy (CCHL) Master Training, an interactive program in which participants will have hands-on learning experience to build better communities.
Post-training, Master Trainers will receive a certificate of completion and will be able to provide training sessions. Master Trainers will be able to facilitate two different trainings: (1) 2 hour CCHL training sessions for workforce professionals and (2) 7.5 hour master training sessions for future master-trainers.

Participants must apply by end of business Tuesday, November 26, 2019. Please note, completing the application does not confirm attendance to this program. Upon completing the application, you will receive follow up from a CCHL lead with next steps.   For more information on the CCHL program click here.

ADULT AND YOUTH MENTAL HEALTH FIRST AID TRAINING 

The Association for Mental Health and Wellness is offering free Adult and Youth Mental Health First Aid Training.  This training is an 8 hour course that gives people the skills to help someone who is developing a mental health problem or experiencing a mental health crisis.  The evidence behind the program demonstrates that it builds mental health literacy, helping the public identify, understand and respond to signs of mental illness.

Substance Abuse and Mental Health Services Administration (SAMHSA) is accepting applications for Promoting Integration of Primary and Behavioral Health Care grants.   The purpose of this program is to:
  • Promote full integration and collaboration in clinical practice between primary and behavioral healthcare
  • Support the improvement of integrated care models for primary care and behavioral health care to improve the overall wellness and physical health status of adults with a serious mental illness (SMI) or children with a serious emotional disturbance (SED)
  • Promote and offer integrated care services related to screening, diagnosis, prevention, and treatment of mental and substance use disorders, and co-occurring physical health conditions and chronic diseases

NATIONAL DIABETES PREVENTION PROGRAM NOW COVERED
New York State Medicaid

The National Diabetes Prevention Program is now a covered benefit for NY Medicaid members.  NYS Department of Health will hold a kick-off webinar on November 13, 2019, 10:00 am to 11:30 am to provide information and details about the new benefit.

FAMILY SERVICE LEAGUE'S DASH CENTER
Offering Those in Crisis an Opportunity to Take the First Step on Their Journey to a Better Life

Every day thousands of Long Islanders face the reality of living with the challenges and stigma of mental illness and addiction. Available to those 5 years of age and above, Family Service League's Diagnostic, Assessment, and Stabilization Hub (DASH) is a unique treatment model and healthcare service that both saves lives and improves quality of life for those in crisis. DASH is Long Island's only 24 hour, 7 day a week, year-round crisis care center.

In the first six months of operation, over 2,500 individuals have been treated. The DASH hotline (631-952-3333) has received over 5,000 calls. The Mobile Response Team (MRT) division of DASH, delivers much needed portable crisis care and has assessed over 700 individuals by responding to locations throughout the community such as schools, businesses and private residents.

NATIONAL DIABETES MONTH   
November is National Diabetes Month, a time when communities across the country team up to bring attention to diabetes.  The U.S. Department of Health and Human Services, in partnership with the National Heart, Lung and Blood Institute, is focusing this year's campaign on the link between diabetes and cardiovascular disease. Use the toolkit available here to learn more and find useful resources, such as videos, infographics, flyers, etc.  Diabet es wellness & self-management resources can be found on SCC's Community Engagement webpage   

LUNG CANCER AWARENESS MONTH
November is a time to raise awareness about lung cancer, highlight why lung health is important and help make a difference. American Lung Association's LUNG FORCE is uniting people across the country to stand together against lung cancer--the #1 cancer killer of women and men--by encouraging them to share their voice.   Additional lung cancer resources can be found on SCC's Community Engagement webpage on the Cancer Screening and Awareness tab  here .  

NATIONAL ALZHEIMER'S DISEASE AWARENESS MONTH
The Alzheimer's Foundation of America is encouraging individuals to be proactive about their brain health and wellness by getting a free, confidential memory screening this November as part of Alzheimer's Awareness Month and National Memory Screening Month.  To find out more about memory screening and who should be screened click here.

NATIONAL FAMILY CAREGIVERS MONTH
The theme for National Family Caregivers Month is #BeCareCurious.  Family caregivers want their loved ones to have the best care possible.  Caregivers are encouraged to ask questions, explore options and share in the care decisions that affect the health and well-being of their loved ones.  The Caregiver Action Network has resources to help caregivers on their website here

NATIONAL HOSPICE AND PALLIATIVE CARE MONTH
This year's theme is "It's About How You Live."  Hospice and palliative care programs across the country are reaching out to raise awareness about hospice and palliative care. Hospice is not a place but high-quality care that enables patients and families to focus on living as fully as possible despite a life-limiting illness. Palliative care brings this holistic model of care to people earlier in the course of a serious illness.  To find more information, visit the National PACE Association's website.  

CORPORATE COMPLIANCE AND ETHICS WEEK
This year will be the 15th annual Corporate Compliance and Ethics Week celebration, November 3-9, 2019.   Recognizing this week is a good way to raise awareness and call attention to the topic of compliance and the importance of maintaining an ethical culture in your organization.  Visit the Health Care Compliance Association's website for ideas, resources and learn how  other organizations are celebrating .

BETTER CHOICES, BETTER HEALTH ® WORKSHOP SERIES
Free 6 week health education program for people with chronic illness.  Ongoing registration and information on various programs offered throughout Suffolk, can be found on the SCC Community Bulletin Board here.  

DIABETES PREVENTION PROGRAM
Free to Suffolk County Residents at-risk for developing type-2 Diabetes.  Offered by Suffolk County Department of Health Services, this program is led by a trained lifestyle coach.  Visit  Suffolk County's website or contact  Deborah at 631-853-2928 for more information and program schedule.

NYS DOH ANNOUNCEMENTS

ELECTRONIC VISIT VERIFICATION SOLUTIONS REQUEST FOR INFORMATION
The NYSDOH has issued a Request for Information (RFI) to solicit input from vendors regarding Electronic Visit Verification (EVV) solutions. This information will assist the State with identifying EVV procedures and systems that address and reflect, to the maximum extent possible, feedback received from stakeholders, while also complying with EVV requirements.   The RFI can be found here.
