A priority of the Network is to serve as a resource to both dialysis patients and professionals throughout PHFPQ activities. Open communication about existing processes, barriers, and successes is encouraged. Network staff members are interested in supplementing resources for what is working in your facility, not adding additional burden. However, in order to assess vocational rehabilitation barriers, and identify how the Network can effectively meet needs of the community, completion of the activities listed below is essential to success of the project.

One assessment per facility.
Through this tool, participating facilities will be asked to provide the name, phone number, and email address of the staff member responsible for implementing project activities, details of current vocational rehabilitation processes, root cause analysis and PDSA results.

Dear ESRD Dialysis Providers,
As you may be aware, the Social Security Administration (SSA) supports the Ticket to Work program that helps people with disabilities progress toward financial independence. Under this free and voluntary program, eligible beneficiaries who are blind or have a disability and receive Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) benefits are entitled to sign up with an Employment Network (EN) or a State Vocational Rehabilitation (VR) agency of their choice. Approved service providers coordinate and provide appropriate services to help eligible beneficiaries find and maintain employment. Appropriate services may include training, career counseling, vocational rehabilitation, job placement, and ongoing support services necessary to achieve a work goal.
IPRO End Stage Renal Disease (ESRD) Network of New England is excited to announce a national initiative that promotes vocational rehabilitation referrals and utilization of services. The Centers for Medicare & Medicaid Services (CMS) 2018 Population Health Focused Pilot Quality Improvement Activity (PHFPQ), Support Gainful Employment of ESRD Patients, is designed to increase referrals EN and/or VR agencies for eligible patients between the ages of 18 and 55, along with increasing the number of patients receiving services from EN and/or VR agencies.
In collaboration with dialysis professionals and patient advocates, the Network will be assisting facilities to overcome barriers in vocational rehabilitation referrals for eligible patients and improve communications between patients, healthcare providers, and EN/VR agencies to facilitate access to appropriate services.
NOTE: Documentation of vocational rehabilitation status in CROWNWeb is vital, as it will be the source of measuring improvement at the facility-level.

Your facility has been selected to participate in the PHFPQ, as the referral rate of eligible patients to vocational rehabilitation services during the baseline period of October 2016 - June 2017 was zero (0) according to CROWNWeb data.

| * Participating facilities shall demonstrate a ten (10) percentage point increase in referrals to EN/VR agencies in by September 30, 2018. * Participating facilities shall demonstrate a five (5) percentage point improvement in the number of patients receiving EN/VR services by September 30, 2018. |
