The SCC held this quarter's Project Advisory Committee (PAC) meeting at a new location, the Hilton Garden Inn at Stony Brook University's West Campus, on September 20, 2018. We were pleased to have Ryan Ashe, MPA, PMP, Director of Medicaid Payment Reform at the NYS Department of Health, kick-off the meeting with his presentation,
True Population Health in the Context of Value Based Payment. He gave an update on Value Based Payment (VBP) progress and spoke about the NYS VBP Model and Population Health, Social Determinants of Health/Community Based Organization VBP Roadmap Requirements and considerations as VBP moves forward.

Representatives from the Family Service League (FSL) provided an overview of their new crisis services for Suffolk County.
FSL's Crisis Services Continuum includes a crisis stabilization program, mobile response teams and a hotline, all operating 24/7. The crisis stabilization program, (Diagnostic, Assessment and Stabilization Hub - DASH) is a comprehensive service that provides crisis care for Suffolk residents who
struggle with mental health and substance use disorders. Care is provided
at a free-standing, community based site at the Hauppauge Industrial Park, serving children (ages 5 and up), adolescents and adults.

Innovation, Bill Bishop, MHA, and Diana Cappabianca, RN, Education and Care Coordinator, from Meeting House Lane Medical Practice, PC, entitled,
Leveraging Regional Health Information Organization (RHIO) Alerts to Improve Outcomes. Bill provided SCC and NYS PPS performance metrics and an overview of RHIO alerts. Diana described her organization's experience in the implementation of RHIO alerts, including challenges, lessons learned and positive outcomes.

MOBILE RECOVERY CENTER - ADDICTION TREATMENT
Family Service League's (FSL) clinical team includes physicians, registered nurses and social workers. They offer comprehensive substance abuse and mental health assessments, diagnosis and medication management in coordination with the development of a treatment plan.
Mobile Recovery increases access to treatment by utilizing peer supports, clinical services and emerging technology to remove barriers which improve individuals' engagement on their road to achieving recovery. FSL coordinates care with substance abuse treatment providers across Suffolk County. For more details about these services,

Matthew McCluskey, Director, Behavioral Health Services Integration
Matt joins us after most recently working at Brookhaven Memorial Hospital Medical Center as Manager, Ambulatory Services with responsibility for administrative, operational, clinical and financial operations for behavioral health and primary care programs. Prior to that he had acquired leadership experience in vocational services operations. Matt brings to his new role a robust background and experience in behavioral health and quality improvement.

The Supportive Housing Network of New York (SHNNY) has released a new video featuring New York State's Deputy Medicaid Director, Elizabeth Misa. The video highlights the Medicaid Redesign Team (MRT) Supportive Housing Program and the significant investment that NYS has made in addressing the Social Determinants of Health.

The NYS DOH is initiating a comprehensive review of the advance care planning process, including any necessary changes to NYS documents (Medical Orders for Life Sustaining Treatment (MOLST), Nonhospital Do Not Resuscitate and Health Care Proxy forms) used during this process. Access
