- $250 for three policies sold.
- $500 for five policies sold.
- $1,250 for 10 policies sold.
Available plans include:
- Premium plan with reduced rates:
- 100% diagnostic & preventive/80% basic/50% major coverage, $2,000 annual maximum, and a $100 deductible.
- Basic plan:
- 100% diagnostic & preventive/50% basic/0% major coverage, and $1,000 annual maximum, and a $50 deductible.
- Enhanced plan
