As the State continues to collect success stories, we want to open the opportunity up to partners to showcase the successful work they've been doing as a result of DSRIP. Stories can focus on new programs or initiatives created as a result of DSRIP, a Medicaid member's perspective on how their care has improved, DSRIP's impact in the PPS' community, the launch or close of an initiative, and/or other transformation efforts. Your stories can be new or old, and previously published stories can also be submitted.
 
These stories will be curated into a story bank called "DSRIP in Action" with a series of success stories from all PPSs across the state, which will be shared various ways throughout the State's network. We also plan to share these stories with our network through our website, newsletter, social media, etc. If you have any pictures or videos that accompany your success story, please feel free to submit those as well.

Healthix Announces Waiving Connection Fees to Support Physician Adoption and Integration with Health Information Exchange

This offer is available for physician practices through September 30. Additional funding incentives of up to $13,000 per eligible practice are also available from the State's Data Exchange Incentive Program (DEIP) and SHIN-NY Connections Initiative (SCI) through Healthix. Learn more about the program here.

Healthix is a public health information exchange (HIE or RHIO) funded by New York State. We electronically collect and consolidate patient health records from +6,000 healthcare providers in New York City and Long Island, providing the clinical information to treating clinicians at the point-of-care.
 
With Healthix, you can: 
  • Look up a patient's data from other provider visits, viewing past history, encounters, labs and more
  • Receive alerts when a patient is admitted/discharged from an ED, hospital or skilled nursing facility
  • Prepare for value-based payments by engaging in the electronic communication requirement of New York State Transformation Programs like DSRIP or PCMH

Community Health Worker Graduate Gets Accepted into CSI Healthcare Management Graduate Program

Beata works at the Community Health Center of Richmond as the Manager of Patient Financial Services. She's been accepted into the Healthcare Management graduate program at the College of Staten Island for the Fall 2018 semester.

Beata started as a graduate 
of the Community Health Worker Training program. Even though Beata has been working in the healthcare field for 18 years, she wanted to continue her education and take away all that she could from the program. Through the CHW program, Beata learned how to control bias, take a step back to see other perspectives, and generally how to feel more comfortable when working through her patients' issues. The hands-on learning and sharing of experiences stuck with her as well because she was able to hear how other people deal with issues and how they handle them. She now takes what she's learned from the program and applies it to her work every day, being proactive instead of reactive. She felt that the CHW program was extremely helpful for her, even in her management position.

Beata plans to continue her education even further past the Healthcare Management graduate program since healthcare is ever-changing, and she always wants to stay up to date. She wants to set the example for her team that you can never have too much education or experience.


*NEW* Buprenorphine Online Enduring Program Launches

The NYSDOH's Office of Drug User Health is officially launching their new online, free "Buprenorphine for Opioid Use Disorder" Enduring Program for prescribers who have completed a buprenorphine waiver training, developed in collaboration with the University of Buffalo and featuring two NY state subject matter experts, Dr. Sharon Stancliff & Dr. Paul Updike. This video presentation offers 2.0 AMA PRA Category 1 Credit(s)(TM) and the opportunity to learn best practices, considerations, case studies and more related to buprenorphine. The Department of Health launched their DSRIP Communications Corner, an initiative intended to provide insights to PPS on what is in the news regarding DSRIP across New York State. Additionally, upcoming events across PPS will be included.

Community Partner Spotlight

HEALTHi at Project Hospitality
A 54-year-old female diagnosed with COPD and Diabetes was living without a usable mattress. She attends her methadone clinic weekly and all scheduled doctors' appointments. She has been clean for 20 years. She's been taking care of her kids and grandkids the best she can. Thanks to HEALTHi, she was able to buy a reasonable mattress for herself. She currently sleeps on one that is stained due to incontinence, and full of dust making her COPD worse. Even a small improvement of a new mattress will help her breathe and sleep better at nights improving her quality of life.
The Community Health Center of Richmond (CHCR) Expanded Their Medical Office Space

Congratulations to the  Community Health Center of Richmond ! They've expanded their medical office
space to the second floor at the 135 Canal St. location. The primary care and dental center features three primary exam rooms and six dental rooms, along with other medical spaces. Community leaders and officials celebrated the ribbon-cutting and opening of an expanded primary care and dental center in Stapleton.

Value Based Payment Corner

Staten Island PPS launched its Value Based Payment Resource Center earlier this year. This resource is hosted on the SI PPS website and supports our overall initiative to educate and prepare the SI PPS Partner Network for the transition from "fee-for-service" to "value based payments," including the SI PPS VBP Education Program.  

The third in-person session of the SI PPS VBP Education Program - VBP Data Analytics - took place at the SI PPS PAC Meeting on June 26th and covered the keys to success for VBP data analytics. We have added a recording of the presentation given by Jason Ganns, the Director of Bureau of Performance Management and Quality Improvement at the NYS DOH, to our VBP Resource Center, found here, along with the supporting document distributed to attendees. We have also added the link to VBP University from the NYS DOH, which is designed to be an academic resource to raise awareness, knowledge, and expertise in the move to VBP.

 Further content will be posted as our curriculum continues to roll out in 2018.
