Empowering families to advocate for themselves so that children with special educational needs can reach their full potential in education and life - through support, training, and education.

Transition | Planning for Post Secondary Success

You've probably heard about transition plans, but do you know what a transition plan is?  The transition plan is part of the IEP process and must be part of a student's IEP by the time a student is 16 years old.  But it's never too early to s tart preparing for the transition process.  At MPACT, we want to make this preparation easier for you!

These fact sheets can help you better understand the transition process:

Another way MPACT supports students and families is with our 22 Regional Transition Networks (RTN's).  Some of these networks host events for students and their families. and this Fall we are hosting 8 transition events.

If you have questions about your child's transition plan or the special education process, please contact us at 800.764.7364.   You can also send us a website submission here.  You can learn about our statewide workshops here.

Remember, MPACT is here to provide information and assistance to empower YOU to be your child's advocate.  If you have questions about your child's education or the special education process, don't hesitate to contact us!
