HCCI Introduces Regional Pilot Program
to Expand Home-Based Primary Care
Florida House Call Project Launches with Call for Applications
In keeping with its mission of making home-based primary care (HBPC) the national standard for treating the nation’s aging, frail, and medically complex patient population, HCCI is launching The Florida House Call Project , with support from the RRF Foundation for Aging . The first program of its kind in HCCI history, the pilot program will ultimately serve as a model for future national research aimed at strengthening the field and practice of home-centered care.
 
Among HCCI’s objectives for the program are to expand the national footprint of home-centered care and grow the workforce of trained, qualified house call providers, with this phase, specifically in Florida. As part of the initiative, HCCI will identify providers and operations professionals in Florida-based practices to serve as Home-Centered Care Champions who will receive training designed to prepare them to spread awareness of HBPC within their communities.
 
Home-Centered Care Champions will serve a 15-month term, during which they will facilitate local education of other providers, practice/operations staff, and the public, using HCCI resources, tools, and curriculum. HCCI will use learnings from this first cohort to evaluate the program model and fine-tune it for potential launching in other markets around the country.

Practice Management Tip of the Month:
Medical Necessity Documentation 101
The medical necessity of a service is the overarching criterion for payment in addition to the individual requirements of a CPT (Current Procedural Terminology) code. Documentation of medical necessity should do the following:
 
  • Identify a specific medical reason or focus for the visit (e.g., worsening or new symptoms)
  • Document the rationale for ordering tests or referrals
  • Describe how the patient/caregiver has managed chronic conditions from the previous visit to present or explain acute symptoms; status of three chronic conditions may be used for History of Present Illness (HPI) credit
  • Include within the assessment and plan the provider’s clinical impression, condition status, and treatment plan for each diagnosis assessed that day
  • Summarize the patient’s health (e.g., improved, worsening, not responding as expected) and document services performed, treatments recommended, medication management, education/counseling, and goals of care conversations
  • Represent the patient complexity, overall patient risk level, and any aggregating factors or psychosocial challenges
  • Document initiation of, or changes in, treatment
  • Include patient and nursing instructions, therapies, and medications
 
Evaluation and Management (E/M) Documentation Requirements
  • Chief Complaint (CC) and HPI
  • Past, Family, and/or Social History (PFSH)
  • Review of Systems (ROS)
  • Exam
  • Medical Decision-Making and Complexity
  • Detailed Assessment and Plan describing the work and treatment decisions
