Requirements for accurate telehealth documentation

To support you in the care of your patients who are Security Health Plan members, we want to ensure your telehealth documentation completely and accurately reflects services rendered.
Your telehealth documentation must, at a minimum, include the following:
- Method of telehealth (e.g., secure two-way interactive video connection, phone call, etc.)
- Member location (e.g., clinic [city/name], home, other)
- Listing of all clinical participants, roles and actions (e.g., applicable when member presents at a clinic with MD at another location)
- Provider location (e.g., clinic [city/name], home, other)
- Time spent, including start/stop times, if required for the service (e.g., psychotherapy)
- Patient consent
Other documentation requirements are the same as with a face-to-face encounter.
