We Need Your Help!
Share Your Experiences with Telehealth

The COVID-19 pandemic has changed the way we do almost everything-including how we receive our medical care.

ASF recently partnered with UCLA to examine the needs of families with serious developmental disabilities. Now we are working with the  Child Neurology Foundation (CNF) to learn more about your experiences with telehealth, including positive and negative experiences, or no experiences at all.

CNF will use this data to provide resources for both caregivers and providers, and help find ways to advocate so that you and your family get the quality of medical care you deserve. If you have any questions about this survey or CNF in general, please email [email protected].

To participate in the survey, click here. This survey will close on July 17.
