Hello Community Partner,


We are contacting your organization from the Community Branch of the City and County of San Francisco's COVID Command Center. The City & County of San Francisco is seeking the input of local organizations to determine options to support the Community during high heat days while we are still living through COVID-19.

This year, as we are all forced to simultaneously plan for extreme heat and COVID-19, the City would like to explore how we can best support these efforts. This support can take many forms. The purpose of this survey is to accomplish two main goals:

1. Assess the needs of San Francisco's community organizations and networks to prepare for and respond to the combined impacts of extreme heat and COVID-19 so the City can effectively support these organizations.

2. Identify organizations and facilities interested in partnering with the City to open congregant cooling spaces for San Franciscans unable to safely stay cool in their own home. City support to this project may include information and guidance, communication support, and resource support including PPE.
