Advancing the Use of Home Dialysis
Transitional Care Units (TCU) are a strategic approach to ensure new ESRD patients are receiving education on modality options, including home dialysis, and other treatment education for success. Have you, your physicians or anyone on the facility care staff considered development of a TCU as a collaborative care model? Use the tool below to learn more about design and execution to develop a TCU including the care benefits for patient education and health outcomes. 
Increasing Patients on the Transplant Waitlist
Providing educational guidance to patients about kidney transplant is valuabl e to jump-start their knowledge in the transplant process. Introduction to important topics regarding multi-listing, referral and suitability for transplant and continued quality of life helps patients as they undergo the transplant journey. Share these transplant resources with patient s going through transplant workup or patients interested in kidney transplant. Assist the patient in writing down any questions they may have for the transplant center staff.

Patient and Family Engagement
Per CMS Guidance, due to COVID-19 pandemic, all patient engagement and advocacy activities such as peer mentoring, attending support groups and participating in Quality Assessment and Performance Improvement (QAPI) and Plan of Care meetings, should be done so virtually, over the telephone, online or thru another distant, non-person to person format to limit patient exposure risk.

The Network recommends facilities continue engagement with your Network Patient Representative to discuss patient needs at your facility. Utilizing bulletin boards and developing activities that engage patients in their care while maintaining social distancing guidelines are encouraged.

The Network has worked with the Patient Advisory Council to develop two new Bulletin Board Kits - Transplant Trains and Blast Out Bugs for Infection Prevention. The kits have been designed so you can print, cut and post in the designs with additional resources in your facility.
