While we are not having an Open House, we want you to know that we are looking forward to meeting the health needs of your students when they are at CMS. Below you will find the process needed to get medication to school.
The student's physician must either write a separate prescription for school or use a district medication form stating the reason for the medication, the dosage, time and frequency that the medication should be administered by school personnel. Parents must also provide signed permission for school personnel to administer the medication.
Prescription medication must be supplied in original labeled prescription bottles, or ask the pharmacist for a bottle for home and one for school each time the prescription is refilled. Non-prescription medication must be provided in the original labeled container.

Prescription medication that is NOT a controlled substance can be brought to the health office by your student on their first day of school; inhalers, rescue medication, EpiPens, diabetic supplies. Please complete and attach this form with the medication.

Prescription medication that IS a controlled substance can be brought to the school by arranging an appointment with staff to allow for the hand-off of medication in the parking lot. Please complete and attach this form with the medication.

Medication that is available over-the-counter can be brought to the health office by your student on their first day of school; Tylenol, Motrin, etc. Please complete this form and attach with the medication.
Note: All medications must be kept in the school health room unless special arrangements are made with the School Nurse.
