It's hard to believe this week marks the halfway point for program year 0 of the Hospital Transformation Program (HTP). A friendly reminder: Midpoint Reports for the Community and Health Neighborhood Engagement (CHNE) process are due
THIS FRIDAY, April 19, 2019 by 5 p.m. Details on how to submit your reports, available data and additional resources can be found below.

We have been getting questions on how hospitals will be evaluated on identified measures for the HTP. On Friday April 12, a draft of the scoring framework was submitted to the Colorado Health Care Affordability and Sustainability Enterprise (CHASE) Board and will be discussed at the next Board meeting on April 23. Additional information about the CHASE Board meeting, as well as measures scoring, can be found in this newsletter. Please don't hesitate to reach out if you have any questions.

Midpoint Reports
The Hospital Transformation Program (HTP) requires hospitals to report on the progress made in the first phase of the CHNE process in their Midpoint Reports, including updates on community engagement and results of their environmental scan of community characteristics, needs, and resources. These reports will be made available to the public by request once the Department has completed its reviews and the reports are finalized around the middle to end of May 2019. We will send out a notification when those are available.
 
The Midpoint Report template is on the HTP website, as well as a link to the Midpoint Report training presentation. An audio file of the presentation is available by request. Midpoint Reports are a good tool to keep in mind as CHNE meetings are being hosted. When convening, hospitals and stakeholders should come prepared with information about the community, including the results of recently completed community needs assessments. Community needs and resources should be compared to the HTP priorities to begin identifying opportunities for initiatives and partnerships.

Data resources for providers
We have provided several data resources to support providers as they put together their Midpoint Reports. In addition to the data resource links on the HTP website, which include data from the Colorado Department of Public Health & Environment and the Colorado Health Institute, the Department of Health Care Policy & Financing (the Department) sent each hospital a series of Health First Colorado data reports, and links to health literacy resources.

Details on submission
Midpoint Reports must be submitted in .pdf form with all supporting documentation included in one document via e-mail
by 5 p.m. on April 19, 2019 to the Colorado HTP email address . Additional CHNE resources can be found on the HTP website.

Quality measures scoring

Last week, a draft document was submitted to the CHASE Board outlining a general framework for how hospitals will be evaluated on identified measures for the HTP. The recommendations for quality measures scoring contained in the draft are intended to guide and inform the decision-making process to finalize the program framework. The process for finalizing this proposal will include collaboration and input from key stakeholders including the Department, the Centers for Medicare and Medicaid Services (CMS), the CHASE Board, the Colorado Hospital Association, participating hospitals, and broader stakeholders participating in the program.

The Department is recommending measure scoring for the HTP include a combination of statewide measures for all hospitals statewide, and local measures selected by each individual hospital to align with their improvement priorities and community needs. Kudos!
Kudos to Pagosa Springs Medical Center! Their HTP presentation to the Upper San Juan Health Service District Board of Directors was written about in The Pagosa Spring SUN . The presentation highlighted the collaborative effort that will take place across the community to give patients the best care in the most appropriate setting.

CHASE Board meeting
The next  CHASE Board  meeting will take place on Tuesday, April 23 at 3 p.m. at 303 E 17th Ave, Conference Room 11abc, Denver, CO, 80203. The meeting is open to the public and can be accessed  via webinar .
