Hospital Transformation Program Concept Paper

Thank you for signing up for the Department of Health Care Policy & Financing Hospital Transformation Program (HTP) Newsletter. The HTP Newsletter will be an important channel for getting information and updates about the program to various stakeholders. As this newsletter evolves, please feel free to reach out and let us know what you'd like to see included in these updates.
 
I'm pleased to announce two new staff members have joined the HTP team! Courtney Ronner, Hospital Relations & Communication Manager, will be taking the lead on HTP communication efforts and Joe Sekiya, Hospital Transformation Project Coordinator, will be working to coordinate our project plan and day-to-day activities on HTP development and implementation. They have been invaluable additions to the program and I encourage you to use them as additional resources. 
 
With the Community and Health Neighborhood Engagement (CHNE) Process in full swing, we are excited to continually hear about the great collaborations, discussions and planning happening in our communities across the state. It is truly amazing to see all of the great engagement happening! As community engagement and partnership are cornerstones of the HTP, we look forward to learning about your continued engagements as well as being a partner in your efforts.

Community and Health Neighborhood Engagement (CHNE) Process
CHNE Action Plans Summary Posted to HTP website
As you know, the CHNE process is intended to build on the strong spirit of collaboration within Colorado's health system and efforts and partnerships already underway. Hospitals that are potential program participants submitted a CHNE Action Plan to the Department that outlines how they will conduct the CHNE process. These plans are available to the public by request and a CHNE Action Plans Summary document has been posted to the HTP website

Those interested in learning more about the CHNE Actions Plans for particular hospitals can request a copy by contacting the hospital directly (contact information for hospital can be found in the table at the bottom of the summary document) or Courtney Ronner, Hospital Relations & Communication Manager, at 303-866-2699.
Invite Us to Your Community Meetings
HTP staff would love the opportunity to participate in your community meetings and learn more about your CHNE process! Eventually, we'd like to create a comprehensive list of all community meetings for historical and transparency purposes. Additional Resources on HTP website
Additional resources have been posted to the HTP website which are of interest to providers and stakeholders:
  • Concept Paper - The Department created a Concept Paper to give an overview of the HTP for the Colorado Healthcare Affordability and Sustainability Enterprise (CHASE) Board.
  • Additional Data Resources - Links to a list of valuable Colorado Department of Public Health & Environment (CDPHE) data have been posted to the HTP website. This information can be found on the HTP website under the "Community and Health Neighborhood Engagement Process" section.
  • Midpoint Report Resources- Through the Midpoint Report, program participants will report on CHNE work over the first half of the pre-waiver process to the state regarding efforts to engage community partners. The deadline for the Midpoint Reports is April 19, 2019.

Information and links to the Midpoint Report Template and Midpoint Report Training Presentation are available on the HTP website (audio for the Midpoint Report Training Presentation is available upon request). This information can be found on the HTP website under the "Community and Health Neighborhood Engagement Process" section.

Data
We have sent two series of data reports to hospitals to support the CHNE process. The first series of reports shared with each general hospital in Colorado include a description of the overall population served in the preceding year; demographics include age distribution, gender, race, housing status, and primary language spoken. Other reported factors encompass category of aid eligibility and Health Program Code.

Subsequent sections of the report focus on specific populations for which value-based care is appropriate, as quality of care can significantly affect costs in these populations. For each of the groups the population served is described demographically, and measures are reported. Measures address top-occurring Diagnosis Related Groups (DRGs) by count and payment, Emergency Department visits, 
Hospital Admits, and Readmission after hospital discharge. 
These reports are provided for the entire population and for those high-utilizer patients that had four or more emergency department visits during the measurement period.

In addition, hospitals have been sent a series of reports with information about particular vulnerable populations who had hospital utilization. The defined vulnerable populations in the report are comprised of people who have:
  1. Cardiovascular Disease (Atrial Fibrillation, Congestive Heart Failure, Ischemic Heart Disease, Stroke)                      
  2. Diabetes
  3. Asthma
  4. Alzheimer's and related Dementia
  5. Bone Disease (Osteoporosis, Osteoarthritis, Rheumatoid Arthritis)
  6. Cancer (Breast, Colorectal, Prostate)
  7. Chronic Kidney Disease
  8. Chronic Obstructive Pulmonary Disease
  9. Hyperlipidemia
  10. HIV/AIDS
  11. Children in Foster Care
  12. Waiver Clients
  13. Pregnant Women
  14. Hepatitis
With each series, hospitals received a report for their hospital, as well as similar reports pulled by the Regional Accountable Entity (RAE) region, and for the entire state of Colorado.
Upcoming Due Date
The next deliverable for the CHNE process is the mid-point report, due April 19, 2019.
