We have sent two series of data reports to hospitals to support the CHNE process. The first series of reports shared with each general hospital in Colorado include a description of the overall population served in the preceding year; demographics include age distribution, gender, race, housing status, and primary language spoken. Other reported factors encompass category of aid eligibility and Health Program Code.
Subsequent sections of the report focus on specific populations for which value-based care is appropriate, as quality of care can significantly affect costs in these populations. For each of the groups the population served is described demographically, and measures are reported. Measures address top-occurring Diagnosis Related Groups (DRGs) by count and payment, Emergency Department visits,
Hospital Admits, and Readmission after hospital discharge.
These reports are provided for the entire population and for those high-utilizer patients that had four or more emergency department visits during the measurement period.
In addition, hospitals have been sent a series of reports with information about particular vulnerable populations who had hospital utilization.
The defined vulnerable populations in the report are comprised of people who have:
- Cardiovascular Disease (Atrial Fibrillation, Congestive Heart Failure, Ischemic Heart Disease, Stroke)
- Diabetes
- Asthma
- Alzheimer's and related Dementia
- Bone Disease (Osteoporosis, Osteoarthritis, Rheumatoid Arthritis)
- Cancer (Breast, Colorectal, Prostate)
- Chronic Kidney Disease
- Chronic Obstructive Pulmonary Disease
- Hyperlipidemia
- HIV/AIDS
- Children in Foster Care
- Waiver Clients
- Pregnant Women
- Hepatitis
With each series, hospitals received a report for their hospital, as well as similar reports pulled by the Regional Accountable Entity (RAE) region, and for the entire state of Colorado.
