To Our District 67 and District 115 Parents and Guardians, 

Our thanks to all who participated in this week’s Lake Forest High School Pfizer teen vaccine clinic. Those who received their first dose of the vaccine will return for dose two on Wednesday, May 26.

In anticipation of Pfizer-BioNTech receiving Emergency Use Authorization from the Food and Drug Administration for its COVID-19 vaccine for children ages 12-15 next week, we are working with the City of Lake Forest, Village of Lake Bluff, and other school districts in our area to schedule a variety of vaccine options for our Lake Forest school community.

Deer Path Middle School - May 16 and June 7
We are working with the City of Lake Forest and Doctors Tests Center to offer the following Pfizer vaccine clinics at DPM. Pending FDA approval, the first dose will be administered on Sunday, May 16 and the second dose will be administered on Monday, June 7.
Deerfield High School - May 16 and June 6
District 67 and District 115 have been invited, in partnership with Walgreens, to participate in a vaccine pod for individuals 12 years and older. Pending FDA approval, the first dose will be administered on Sunday, May 16 and the second dose will be administered on Sunday, June 6. The clinics will be held from 8:00 AM to 6:00 PM at Deerfield High School, 1959 Waukegan Road, Deerfield. 

The portal to schedule appointments for these clinics is now open. Each vaccine recipient must have an appointment. 
Please note the following important information. Additional information including information about parking is available at this link: vaccination.dist113.org.

  • There is no cost to receive the vaccine. 

  • FDA EUA approval is anticipated and therefore availability of vaccine is contingent upon FDA authorization. 

  • Children receiving the vaccine must be age 12 on or before May 16, 2021.

  • The Lake County Health Department and Walgreens require that a parent/guardian be present for vaccine appointments for children ages 12-17.

  • Adults ages 18 and older can provide consent and do not need a parent/guardian present.

  • Vaccine recipients should bring an insurance card if you have insurance and a government-issued photo ID. For minors, the parent/guardian should bring the insurance card and government-issued photo ID. 

  • If the vaccine recipient is receiving only the second dose of the Pfizer vaccine on either date, you must bring your CDC vaccine card to your appointment. 

  • The Walgreens consent form is required for each vaccine recipient at each appointment and it must be printed on one sheet of paper, double-sided. To make your appointment go as quickly as possible, the form is attached here so that you can bring a completed, double-sided form with you. If you do not have a printer or cannot print double-sided, forms will be available onsite. 

  • All health and safety guidelines will be in place; face coverings are required.
