Holiday Closure
Our offices will be closed on Monday, December 24 and Tuesday, December 25. We wish you and yours a peaceful holiday! We will also be closed on New Year's Day.

Flu Shot Reminder
QualChoice members can get the flu vaccine at any network pharmacy or provider's office. No copayment is required.

Moving to Value-Based Care

As well as Comprehensive Primary Care Plus (CPC+), we are proud to participate in the statewide Patient-Centered Medical Homes (PCMH) program, developed to improve patient care and population health, and to lower healthcare costs. Youth Home helps youth, adults and families become healthier contributing members of the community by providing compassionate emotional and behavioral health care. The Dove Tree program provides gifts to residential patients. In December, volunteers help wrap donated gifts.
