Hello Everyone,
As I write this, the rain is pouring down in buckets and has been for quite a while. I hope everyone is safe and dry, and looking forward to the beautiful fall weather and colors that are on the way.
Well, today I just feel compelled to discuss something again that I wrote about not too long ago. We have had a significant amount of Immediate Jeopardy situations related to Elopements in the recent months. In many of those cases, but not all, staff heard the alarm go off and turned it off without actually going outside to check for residents or without doing a head count of residents in the facility, or both.
Had facility staff simply performed the actions above, these situations may not have resulted in Immediate Jeopardy citations. The staff would have appropriately responded to the alarm system put in place to ensure resident safety and would have been able to bring the resident immediately back into the facility, thus demonstrating that the system put in place to ensure resident safety was effective.
You may wish to reinforce with your staff the need to respond to door alarms promptly and to ensure that no one has gone outside by actually searching outside and/or doing a head count in accordance with whatever your facility policy/procedure says to do. Sometimes a simple reminder and word to the wise can prevent a tragedy, or at the very least, an Immediate Jeopardy deficiency that you do not want to receive.

Quality Care From the Perspectives of People Living With Dementia
|

One of the most important aspects in CMS’s National Partnership to Improve Dementia Care in Nursing Homes was the importance of utilizing “Person-Centered Care” techniques in caring for residents in long-term care facilities who are diagnosed with Alzheimer’s Disease and various other types of dementia. Several months ago, the Office of Inspector General was granted permission by the Alzheimer’s Association to share a number of their articles that address this subject. The
article
we have chosen for this month’s newsletter has as its central theme the need for providing “Person-Centered Care” in managing dementia. What makes this article so unique is that it discusses these techniques from the perspective of what individuals living with dementia desire and expect from their caregivers. The article illustrates that residents do not necessarily agree on which interventions are most important to them with regard to the treatment of their dementia. Consequently, it illustrates that care for an individual with dementia needs to be very “individualized”. We thank the Alzheimer’s Association for permission to share this article with our readers with the hope it will be informative to both surveyors and caregivers in our long-term care facilities.

Validation Techniques Seminar

Naomi Feil is the Founder and Director in Chief of the Validation Institute. Mrs. Feil developed the Validation technique which is a tested model of practice that helps those diagnosed with Alzheimer’s and related disorders reduce stress, enhance dignity and happiness. Through Validation techniques we are able to enter the person’s inner world.
Everyone is welcome - advocates, families, providers, community members, guardians, AAAILs, social service workers, surveyors, students. We hope to pack the house!

Patient Navigation Program
The Centers for Medicare & Medicaid Services (CMS) has launched a program for people with Medicare that could be useful for your skilled nursing facility. It’s called the Patient Navigation Program and is administered by Beneficiary and Family Centered Care Quality Improvement Organizations (BFCC-QIOs). KEPRO, the BFCC-QIO for our state, has established a Patient Navigation program as part of its Person and Family Engagement (PFE) initiative, to coordinate with the goals of CMS.
KEPRO would like to partner with skilled nursing facilities to offer this free program to Medicare Fee-for-Service patients who are being discharged from a skilled nursing facility to home. This program offers follow-up to patients as they transition to home. This can benefit discharge coordinators and social services staff as they prepare patients to go home during the discharge process by saving time and making their job easier. Some of the services that the Patient Navigation program provides include:
- Assisting with finding transportation options to the doctor’s office for an appointment.
- Helping to identify community resources that a patient may need after discharge.
- Helping patients and families understand their diagnosis and treatment plan.
- Care coordination to help patients access needed health care.

Payroll Based Journal (PBJ) Release Highlights
Please review the information below for important PBJ report and data submission updates:
As of September 9, 2018, users will not be able to update or add staffing hours data that is less than or equal to Fiscal Q2 2018 (January 1, 2018, thru March 31, 2018). This data will, however, still be available for viewing on CASPER Reports. This change does not affect any of the deadline rules stated in the PBJ Policy Manual or online. Please see the PBJ Policy Manual for information about PBJ quarterly submission deadlines. Note, data changed after a deadline is not used by CMS.

The MDS Validation Utility Tool (VUT) v2.2.0 is now available on the QTSO website below. This release is effective October 1, 2018, and supports the v2.02.1 MDS Data Submission Specifications and associated Errata. For additional information please review the associated MDS VUT ReadMe text file.

Update for jRAVEN 1.5.0
A software defect has been identified for jRAVEN v1.5.0 which negatively impacts the ability to complete items A2400B & A2400C for (NT) Nursing Home Tracking records with a Target Date of 10/01/2017 or greater where A2400A = 1.
If you haven’t already contacted the QTSO Help Desk for the update,
please contact the QTSO Help Desk for detailed instructions regarding the remediation of this known defect.
