First of Year Paperwork

We need the following first of year paperwork returned electronically. Please complete the paperwork using the fillable forms below. Save each form with your students Last name.First name and form type.pdf

You may either mail or fax your forms to the school on 859 384 5049 or via email with your students name in the subject of the email.

Annual Student Directory Information Notification

At some point during the school year, school/District personnel or other District-authorized persons may videotape or photograph classroom activities or special projects in which your child participates during or after the school day for staff/student educational or public awareness purposes. Such videotapes or photographs may be viewed by peers, faculty or administrators. On special occasions such as a videotape or photograph of a class or school play or of an academic or athletic event, the film or photograph may be viewed by a general audience including, but not limited to, publishing pictures in yearbooks, event programs and newsletters, or on the school or district web site, as well as, social media platforms.

Although we will make efforts to comply with your request, bear in mind that we cannot monitor all adults at all times, especially during the special occasions when other parents may take pictures or tape the event.

This form also includes an opt out for armed forces recruiters information. You would only need to return this form should you not wish for your student''s information be shared with armed forces recruiters.

Each year parents can choose to withhold their

student's information from institutions of higher education, potential employers, and the military.

Only complete this form if you want to withhold your student's information. If you choose to complete the form, fill it out completely, save it under your student's name (lastname.firstname.pdf), and email it to [email protected] with your student's name in the subject of the email. You may also print out the form and fax or mail it to the school.

Household Change Form

If any of your household information has changed please let the school know by completing the Household Change Form. These forms can be emailed to [email protected] or faxed to the school.

How to pay fees online

Please click here for instructions on how to pay fees online in IC Parent Portal.

School fees will be done virtually this year through our Infinite Campus Parent Portal using a credit card. There is a nominal fee to pay online by credit card.

The average cost of school fees for each student is $118. It could be more based on the courses your student has selected.

Common fee items include (but not limited to):

General Fee: $55 (includes Technology)

Textbook Rental Fee: $8 per class

Workbook Fee: $10

Parking: $15

Science Lab and Art Fee: $15

Choir Fee: $15

If you do not have your Infinite Campus Parent Portal activated, please call our main office (859-384-5040). This will provide access to your students grades, attendance and more.

Transcripts

SENIORS ONLY: Your Parchment registration code has been emailed to your school student email address. Please check your email and use this code to set up your Parchment account. This allows you to send your transcript to any college of your choosing during the application process.

Immunizations

Parents/guardians please make sure to present an updated immunization certificate to the school before returning. Keep in mind that your student must have a second meningitis on or after their 16th birthday, and 2 Hep A's spaced 6 months apart. Check for a correct expiration date. Even if your student is virtual/remote, it is still a state requirement to have an updated certificate.

Parents always want to know what their son or daughter can do to position themselves to get into their top college or to qualify for top scholarships. In this popular workshop, Dan will dive into what it takes today to help every student have a successful college admissions and funding process and HOW COVID-19 has changed the rules of the game.
