Population Health Summit: Innovation under the Maryland Model

Tuesday, Dec. 4
9am- 4pm
The one-day summit will bring together stakeholders from across the state to develop and discuss a statewide framework to address population health. Through statewide focus on specific population health goals, Maryland aims to incentivize statewide investments and engagement in both health system transformation and public health interventions to improve population health.
Registration is now open for Maryland's Office of Minority Health and Health Disparity's  15th Annual Health Equity Conference. The conference will be held on Thursday, Dec. 6 from 9 a.m. to 4 p.m. at Martin's West 6817 Dogwood Road Baltimore, MD 21244.

Funding Opportunity: The Healthcare Initiative Foundation's  FY2019 Capacity Building Grants

The FY2019 Capacity Building Grant round of the Healthcare Intiative Foundation is now open.  

The Capacity Building Grant priorities aim to promote sustainable, culturally competent, integrated healthcare access throughout the lifespan through: (1)  Behavioral Health Across the Lifespan; (2) Access to Quality and Comprehensive Health and Wellness Services; (3) Sustainable Business Models and Integrated Service Delivery Systems to Stimulate Systemic, Positive Change in the Health and Wellness Sector; and (4) Develop a Culturally Competent and Highly Skilled Health Sector Workforce. 
Applications are due by Friday, December 7th at 5pm.

CHRC issues FY 2019 Call for Proposals

The Maryland Community Health Resources Commission (CHRC) released earlier this month the FY 2019 Call for Proposals, which looks to support programs that will serve vulnerable populations; promote health equity and address the social determinants of health; and support community-based projects that are innovative, sustainable, and replicable.

Letters of Intent will be due by noon November 13, and f ull grant proposals will be due by noon December 17. Grant awards are expected to be made by the CHRC in February 2019.

The CHRC is hosting a public information call for potential applicants on Tuesday, October 30 at 10 am (Dial in number: 1.866.247.6034, Access code: 4102607046). Following the call, the CHRC will post a "Frequently Asked Questions" document for potential applicants.

This year's Call for Proposals looks to support three categories of programs:
(1) Promoting delivery of essential health care services: primary/preventative care services, dental services, and women's health care services;
(2) Addressing the heroin and opioid epidemic through integrated behavioral health service delivery; and
(3) Promoting food security and addressing childhood and family obesity.

CHRC grantees highlighted at the annual conference of the Maryland Rural Health Association

The Maryland Rural Health Association (MRHA) hosted the 2018 Maryland Rural Health Conference earlier this month in Cambridge. The conference, " Maryland's Rural Health Roadmap: Pathways to Creating Healthier Communities," invited experts on the national, state, and local levels to offer their perspective on the key issues involved in rural health, economic development, workforce, and social determinants of health. CHRC invited to present on School-Based Health Centers to the Kirwan Commission

The CHRC was invited earlier this month to present to the Commission on Innovation and Excellence in Education, also known as the Kirwan Commission. This presentation provided an overview of CHRC grants supporting school-based health centers and described the work of the Council on Advancement of School-Based Health Centers, which is staffed by the CHRC. The Kirwan Commission brings together experts in education from across the State and country and will make recommendations for improving education in Maryland through funding, policies, and resources. The Commission strives to prepare Maryland students "to meet the challenges of a changing global economy, to meet the State's workforce needs, to be prepared for post-secondary education, and to be successful citizens in the 21st century." The Commission will submit its final recommendations report to the Governor by December 31, 2017.

Frederick Memorial Hospital utilizes CHRC dental grant to reduce ED visits by 64%

Frederick Memorial Hospital (FMH) opened The Monocacy Health Partners Dental Clinic in 2016 to provide oral health care services for individual who visit the hospital ED for dental related conditions. The program was supported by a three-year grant from the CHRC and involves a partnership between Frederick Memorial Hospital and the University of Maryland School of Dentistry (UMSOD). Since the opening of the dental clinic, the number of individuals seeking treatment from dental conditions at the hospital's ED has declined 64%, from 901 in 2016 to 328 in 2018. The decreases in ED usage were seen across age groups.


The Commission has awarded 39 grants totaling $7.8 million to expand access to dental services. These grants have supported programs in 15 jurisdictions. For more information about CHRC's dental health grants, click  here .
