We are requiring that every family fill out a screening form for each student on the first day of each school week. See below for both app and ParentsWeb instructions.
APP INSTRUCTIONS
When you open the app, you will click on WEBFORMS.
Next, you will click on COVID RESPONSE FORM - FAMILY
You will then click on the COVID RESPONSE FORM BUTTON to fill out the information on the first day of every school week.
Please fill out a separate form like the one below for each child listed.
