the Capital Area Summit will feature an Advance Care Planning (ACP) track featuring best practice breakout sessions with statewide ACP experts. Network and learn from
local, state, and national leaders!
Presentations will help attendees understand recent policy changes, advance directive storage/retrieval mechanisms, and how to promote shared decision making at all stages (including community based ACP programs). These presentations will enhance healthcare provider’s ability to honor a person’s wishes!

Do you or someone you care about struggle with memory loss?
Have you ever wondered what kinds of future healthcare
choices someone with memory loss faces?
This class is appropriate for individuals/families concerned about current or future memory loss issues in themselves or a loved one. The presentation will include the basics of brain structure and function, behaviors one may see in dementia, and ways to seek support. In addition, participants will have the opportunity to review, and possibly complete, an advance care planning document that would list their preferences for care in the various stages of memory loss.

To some, talking about funeral rites and passages are taboo.
To others, the conversation is a source of curiosity, practicality and/or reflective of one’s own spirituality. If you have had an advance care planning conversation with us, you also know we ask about your burial preferences in order to make it easier for the ones you leave behind.
This class will answer all sorts of questions about the somewhat newer (and some ancient!) practices of home funerals and natural burials. From the definitions of the various burial choices (including water/marine reef placement) to legal requirements, environmental impact and involvement of family and friends, this is an evening you won’t forget!
