Over 30 PAC Members, SMEs and patients voted on the top 15 submissions.  There was a final tie breaker, and BASSETT HEALTHCARE LITTLE FALLS DIALYSIS came out as the champions!

Preparing for 2019
September's feedback will be the last reporting month for the 2018 Home Therapies and Transplant QIAs.

We will be re-launching the QIAs in January 2019, with a focus on Collaboration, Sustainability and Innovation.  Please take this time to review and report your final numbers and feedback for January - September, 2018, along with suggestions for improving next year's QIAs!

Presentation Objectives:
  • Identify patient referral process.
  • Define first initial phone contact with a patient.
  • Outline transplant work up process.
  • Highlight importance of care partner support.
  • Discuss transplant recipient expectations throughout work up and once listed.
  • Identify barriers/hurdles patients may have prior to starting transplant process.
  • Discuss listing process, barriers, and requirements. 
Project Goals
Transplant QIA: Participating facilities shall demonstrate a 10% point increase in patients who are on a transplant waitlist by October 2018.

Home Therapies QIA:  Participating facilities shall demonstrate a 10% point increase in patients who are training for Home Therapies (HHD or PD) by October 2018.
