Empowering people with disabilities to lead independent
and inclusive lives in the community.
A Note from Our Executive Director
Dear Friends:
1,121 people with disabilities - this is the number of consumers that were assisted through our services as a provider for the Ohio Department of Medicaid HOME Choice Program since 2009. The HOME Choice Program was developed to provide extra support and help with housing to transition eligible Ohioans out of nursing facilities and into home and community-based settings. With the end of the state's iteration of this federal program, HOME Choice Transition Services for CILO concluded on June 30th. To learn about these changes and more visit this link.
Over the years our Transition Coordinators worked closely with these consumers and their families - from infants through seniors. Statewide, more than 13,000 people were successfully transitioned from long term care through the HOME Choice Program. CILO is proud to have been part of this transition program that so closely aligned with our mission to empower people with disabilities to lead independent and inclusive lives in the community.
My thanks to the Ohio Department of Medicaid for their confidence in us to be a HOME Choice provider for the last ten years; and my thanks to those members of our team, both present and past staff, who worked as HOME Choice Transition Coordinators. We should take great pride in all that has been accomplished for our consumers and all of the successful outcomes we have facilitated.
I look forward to continuing to share more successful outcomes throughout the year. As always I want to hear from you! Email me or call me at 513-241-2600 Ext. 211.
Regards,
Rob
