Note from the Nurse:
First, let me say that I miss seeing all of you, students & parents. I always joked around that I'd love just one day of no lines in the Nurse's office, but this is certainly NOT what I wanted!!! Now is a great time to email me those physical exam, dental exam, or immunization forms you have at home. My email is
coaryl@tesd.net.
Just a reminder:
Thus far, no PA state regulations have changed regarding immunizations, physical exam & dental exam requirements.
For Rising 5th Graders going into 6th Grade
:
PA school health law states that children entering 6th grade must have physical examination completed & on file in the nurse's office. Please provide documentation to the nurse ASAP upon completion of the exam.
For Rising 6th Graders going into 7th Grade:
PA school health law requires that all students MUST have the following vaccinations PRIOR to the first day of 7th grade or risk exclusion:
- 1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
- 1 dose of meningococcal conjugate vaccine (MCV)
If your student has NOT had these vaccinations but, is not due for a well visit, please call your health care providers office & request a nurse visit specifically for immunizations. These appointments are typically quicker to schedule & are often at no added cost to you. Other options to obtain these vaccinations include the Chester County Health Department, many area Urgent Care locations, or clinics within pharmacies. Call first & check to see that they have what you need.
Additionally, a dental examination is also state required for 7th grade. Forms to be completed by your family dentist can be found on the TESD website under Student Health Services (See below).
***All health-related forms that a parent/guardian may need for the first day of school are now available to print from the TEMS website:
- Go to TEMS website
- Click on For Parents tab
- Choose Student Health Services from the drop down
- Once on the Student Health Services page if you need forms for medications:
