Dear Parents,

The safety and welfare of your student is of the utmost importance to Northland. In order to ensure this, certain health records must be in order and maintained.

Immunizations
  • Immunization records must be up to date. Click the following link to see the state requirements: 2018-19 Texas Minimum State Vaccine Requirements for Students K-12. If you know that you have not submitted updated information, please do this as soon as possible. I will be auditing files and contacting you if there are any deficiencies.
  • If a family claims an exemption for reasons of conscience, a signed affidavit must be obtained from the Texas Department of State Health Services and the original must be presented. This must be updated every two years.

Screenings
  • Vision and Hearing screenings are required for grades Pre-K, Kinder, 1st, 3rd, 5th and 7th and all new incoming students.
  • Spinal screenings are required for all 5th and 7th grade girls and 8th grade boys. Please email me if your student is already receiving treatment for scoliosis.
  • Please see the attached schedule for the screenings for each grade.
  • I will only contact you if your student does not pass the screening. Please understand that this is for screening only and diagnosis of a problem would have to come from your doctor.

Schedule for Hearing and Vision Screenings
  • Pre-K: September 24
  • Kinder: September 28
  • 1st grade: October 2
  • 3rd grade: October 2
  • 5th grade: September 28
  • New ES students: September 24
  • 7th grade girls: October 3
  • 7th grade boys:  October 9

Schedule for Spinal Screenings
  • 5th grade:** TBD
  • 7th grade girls:** October 3
  • 7th grade boys: October 9
**  For 5th and 7th grade girls spinal screening they will need to wear a sports bra or camisole on that day. They will be required to remove their uniform shirt for the assessment.
