Patients will receive welcome messages, surveys, important information and education about current medications while they are in the hospital. Upon discharge, some patients will also receive post-discharge care plans specific to their condition.

CareNotify is an industry-leading mobile platform for patient engagement, and has previously been launched at Newark Beth Israel and Monmouth Medical Centers.

Clinical Documentation Tip of the Month
"Complete immobility due to physical or medical disability = Functional Quadriplegia"
  • Complete immobility due to severe physical/medical disability or frailty
  • Not true paresis, it is the inability to move due to another condition
  • Person functions as a quadriplegic without a spinal cord injury
  • Severe dementia is the most common underlying cause
  • This person may have pressure ulcers, contractures, and are considered "total care," "bedbound," "max assist" and need assistance with all ADL's
Functional Quadriplegia significantly impacts the patient's severity of Illness (SOI) and risk of mortality (ROM). This diagnosis is considered a Major Co-morbidity and these patients are resource intensive.
