ASQ-3 Screenings
ASQ-3 screening results will be coming home over the next couple weeks . Please note that fine motor ability is often the last area to develop with a preschooler. Do not be alarmed if your child's scores are lower in this area. The large muscles at the center of the body develop before coordinated movement of the fingers. You can help at home by providing many activities to practice using fingers and wrists. Beading, working with playdough and clay, legos, stickers and many other activities can help build the coordination in the fingers necessary for writing, coloring, cutting, buttoning, zipping and other PK skills.

Enrollment Paperwork
All students must have a complete file within 30 days from the first day of school. Items in BOLD below must be completed on the first day of school.
1. current physical (valid for 12 months)
2. immunization records
3.
registration paper (please fill out ALL portions of the dentist, physician and
emergency contact information including
addresses
and phone numbers
)
4.
baptismal or birth certificate
5.
any custody paperwork that we need to be made aware of
6. Dentist form -Does not need to be signed by a dentist
7. Publicity Form
8. An emergency care plan for any children diagnosed with severe asthma, food or insect
allergies requiring an EPI-pen, medical concerns requiring medications to be administered at
school or other health related concerns that would require our faculty to have a plan in place
in the event of an emergency. Please ask your doctor for a form or call the school office.
Please attach a picture of your child to the form.

Please note that while we would love to accommodate "make up days" for days off due to illness, vacation, holiday, teacher work days, snow days and other days off, we are unable to do so. We must maintain ratios and our student rosters that we have for every moment that your child is in school.
If you need to add a day due to a family circumstance, please call the office. Added days are $25 per full day/ $15 per half day. Added day are accommodated when space is available in a room.
There will be times during the year when all students will be invited to attend school for parties, field trips and other events. More information on special occasions will be coming soon.

Most Blessed Sacrament Parish
Most Blessed Sacrament Parish is hiring! Most Blessed Sacrament is seeking a part-time weekend Custodian. This involves maintaining the buildings and property of the Parish, which includes cleaning, grounds maintenance, equipment maintenance and repair. It also requires the ability to troubleshoot and think independently while working with the parish staff, school staff, and our Most Blessed Sacrament community.
