Mount Sinai PPS Supports ACMH Efforts to Offer Short-Term Stay for Patients with Mental Health Crisis ACMH

In recent months, Mount Sinai PPS has been working collaboratively with ACMH, Inc. to develop a process for MSPPS partners to manage patients experiencing a mental health crisis. This collaboration also includes the expansion of ACMH's new short-term crisis respite location in Washington Heights, in addition to the existing locations in the East Village. ACMH's Short Term  Crisis Respite Program offers patients temporary residence with daily living assistance and around-the-clock support from peer counselors. 

The goal of the crisis respite program is to serve as an alternative to an avoidable ED visit or hospitalization. It is also designed to help patients transition from hospitalization and into

Continuing Clinical Implementation Work 

With the start of the new DSRIP Year in April 2018, Mount Sinai PPS has been working with select groups of partners on the following implementation activities to align with its three clinical focus areas of chronic disease management, increasing access to preventive care, and care transitions, coordination, and management. 
  • Care Management Agency Pre-Visit Planning: Last fall, MSPPS care management agencies (CMAs) formed a workgroup to share and discuss best practices around care gap closures. With the assistance of the PPS, they collaborated on a Pre-Visit Planning Tool, a form with a series of questions to engage the patient, care manager, and provider in the patient's care.   • Tobacco Cessation: MSPPS is working with a cohort of partners to plan and implement tobacco cessation programs and activities to align with New York State's reporting requirements and the PPS' clinical strategy around tobacco cessation. To learn more about the work, please click here.

Using Community Gateway's Analytics Application to Improve PerformanceAnalytics 

Mount Sinai PPS is currently working with a small group of partners to utilize the Analytics tool, an application within Community Gateway, our secure portal, to track partner utilization and performance data. This application  aggregates data from multiple sources,   including selected partners' electronic   health records and claims data from New York State's "Salient" database.   
The goal of  this application is to provide partners  with a one-stop-shop   to analyze its  data   to quickly identify areas for quality  improvement. 

Participating partners will have access to visual dashboards and graphs to monitor performance on DSRIP measures, view health home participation data, and identify patients with open care gaps and those who may be high utilizers. This information will help inform strategies around care coordination and clinical operations, while also helping MSPPS forecast partner performance with DSRIP measures.

Partner Spotlight Series: Gay Men's Health Crisis 

In 1981, 80 men informally joined together to raise money for what the Centers for Disease Control and Prevention (CDC) labeled as "gay cancer," thus begin ning the formation of Gay Men's Health Crisis (GMHC).

Since its inception over 36 years ago, the organization has provided patient care to several client demographics that extend beyond its founding name. Dr. Bill Bracker, Senior Vice President of Programs and Clinical Services, said, "Our name is historic because we were the first agency to respond
to the HIV crisis in the world, but while we've maintained that name because it's where our roots are, it doesn't entirely reflect the clients we serve." And being a part of DSRIP has certainly encouraged GMHC to continue to identify innovative ways to reach these broader populations. 

Exploring New Ways to Provide Better Patient Care 
"DSRIP provides us with an opportunity to work on some efficiency projects that we would have liked to do, but didn't have the resources to do," said Ailish Bateman, GMHC's Senior Director of Data and Evaluation. 

For example, GMHC has implemented processes to help track client enrollment in mental health programs and understand client needs more effectively. Within the past year, GMHC has started to use the Patient Health Questionnaire PHQ-2 and PHQ-9 screening tools to assess appropriate client treatments. Clients coming in through the central intake department receive the PHQ-2 screening, and if a patient receives a positive score, he/she is routed to GMHC's mental health department. These clients later receive the PHQ-9 screening and can be referred to a psychiatrist based on the test outcome. Staff will then follow up with clients who scored at a certain level to make sure they receive the appropriate treatment.

As GMHC continues to implement new initiatives to align with both DSRIP and MSPPS goals, it hopes to leverage these new ideas to different facets of its organization that may not necessarily be touched by DSRIP. Dr. Bracker said, "Our impact is quite a bit greater than the patient attribution assigned to our organization, and the kind of work we do helps make sure people's conditions are being treated properly-helping us support the DSRIP vision."  To read the full article, please click here.
