Southwest Network transition

In an effort to broaden the range of service opportunities for members and enhance their choice, effective May 18, 2018, three Southwest Network (SWN) clinics - Mesa Heritage, Royal Palms and Highland - will transition to other network providers with Mercy Maricopa Integrated Care.  The locations of these clinics are as follows:

Mercy Maricopa and SWN are working closely to transition all enrolled members and families to identified contracted providers, or to the participating provider of their choice anywhere in the Mercy Maricopa service area.

We are specifically working with our contracted providers, Community Partners Integrated Healthcare, Lifewell and Community Bridges in order to ensure a smooth transition for members and families.   Mercy Maricopa is reaching out to providers to discuss any operational challenges and help with the transition. Providers are encouraged to contact Mercy Maricopa with any questions or concerns about transitioning SWN members and families.

Member and family outreach  
In addition to regular face-to-face contact, members and families should have already received a letter informing them of the transition. The letter informs members that they can work with their clinical team and/or Mercy Maricopa Member Services to change to any provider in the Mercy Maricopa network at any time, for any reason. It also informs members and families that if they don't select a new provider, they will be assigned to a provider.
 
It's important that we all work together closely to successfully transition members and ensure that they receive the high-quality, comprehensive care that they need and deserve.
