RESOURCES:

The Network is working with 20 facilities, representing 10% of the Network 1 service area. The Network aims to improve patient education about vocational rehabilitation (VR) and employment networks (EN), regular elicitation of patients' goals, and genuine efforts to achieve them. Moreover, as a result of the QIA patients will better understand the benefits of goal setting, VR and EN services, and how to utilize those services.

NOTE: Documentation of vocational rehabilitation status in CROWNWeb is vital, as it will be the source of measuring improvement at the facility-level.

Tools & Resources
Throughout this project, participating facilities will receive tools and resources for patient self-management, goal setting, and VR/EN patient education.

These powerful materials were created to support the activities of Employment Networks (ENs), Vocational Rehabilitation (VR) agencies and other SSA-approved disability employment-focused organizations as they assist people with disabilities as they enter or re-enter the workforce.

To assist you with compliance in this project we
are providing you with a checklist of current activities your facility should be engaging in to
meet the goal:
  
Facility Project Lead Checklist:
  • Review your current census to determine how many patients are eligible (ages 18-54)
  • Talk to your facility's selected Patient Ambassador to discuss their role within the project. ** It is never too late to identify any patient who is interested in serving as the Patient Ambassador (PA).
  • Provide your PA with resources and the list of eligible patients to assist with distribution.  Hard copies will be sent to your facility as well.
  • Download and print the patient stories as a secondary resource for you and your PA to hand out.
  • Create a schedule to distribute resources between for yourself and your PA.
  • Save the date to attend the National LAN call scheduled for June 67th, 2018 at 3pm EST. (registration link in left column)
  • Create a process in your facility to ensure voc rehab information is kept up to date in CROWNWeb for all patients.
  • Review progress with your PA and invite your PA to report interventions, success and barriers in the facility QAPI meeting.
  • If you haven't already, contact your local VR/EN agency to set up an awareness activity for your facility such as hosting a lobby day, inviting local VR/EN agencies to provide information to patients and staff or assembling a bulletin board using Network and local VR/EN materials
Patient Ambassador Role: 
  • Distribute the Network resources and patient stories on vocational rehabilitation.  All resources are listed on the left hand column 
  • Provide interested patients with the contact information to the state level agencies. Help with facility awareness activity planning as noted above.
  • Provide report of your actions and observations at the facility's QAPI meeting.


 

 
Project Goals

* Participating facilities shall demonstrate a ten (10) percentage point increase in referrals to EN/VR agencies in by September 30, 2018.

 

* Participating facilities shall demonstrate a five (5) percentage point improvement in the number of patients receiving EN/VR services by September 30, 2018.
