The next iteration of the Accountable Care Collaborative (ACC) is scheduled to begin July 1, 2018. The goals of the next phase are to improve member health and reduce costs.
In November 2017, the Department announced the awards for the seven Regional Accountable Entity (RAE) contracts.

Update on Our Progress
Contracting & Attribution
The RAEs are currently working to execute their initial round of Primary Care Medicaid Provider (PCMP) contracts with eligible primary care providers before April 15, 2018. While the program will not begin until July 1, the Department requires this information in advance to process member attribution and enrollment by May 15, 2018. This will enable the Department to start notifying members of their RAE assignment by June 1, 2018. Get more information

The Department is currently working with
the fiscal agent, DXC Technology, to test the attribution processes and other enrollment functions. This new functionality will allow the RAEs and providers to review and monitor their member panels. To learn more about attribution, check out our

Performance Measurement
The Department is working with its vendors to finalize the performance measure specifications. The Department's data analytics vendor, IBM Watson Health, will incorporate these measures in to the Data Analytics Portal so PCMPs and RAEs can view their performance monthly. Centers for Medicare and Medicaid Services
The Department is in the process of responding to specific questions from the Centers for Medicare and Medicaid Services (CMS) regarding the Waiver and RAE contracts to ensure approval by July 1, 2018. Read Section 1915(b) Waiver Proposal Narrative here.

Attribution-Common Terms Defined
Enrollment:
Enrollment is the term the Department uses for registering members into the ACC.
Assignment:
Assignment is the method used to connect a Health First Colorado member to their RAE. For Phase II, the PCMP attribution will also determine a member's RAE assignment. Members will be assigned to the RAE region in which their PCMP is located.
Attribution:
Attribution is the method used to link Health First Colorado members to their medical home, or PCMP.
Auto-attribution:
Auto-attribution occurs when a Health First Colorado member is systematically assigned to a PCMP.

Provider Contracting
In ACC Phase II, the RAEs are responsible for developing and managing a network of primary care physical health providers and behavioral health providers to ensure access to appropriate care for Health First Colorado members.
All primary care providers who want to serve as a PCMP must contract directly with the RAE(s). The Department will no longer have a unique PCMP contract with providers.
Behavioral Health providers will need to contract with and be credentialed by the RAE to receive reimbursement for providing capitated behavioral health services as of July 1, 2018. Behavioral Health providers must be enrolled as a Health First Colorado provider before they can be credentialed by a RAE.
Note: Provider contracting with the RAE is separate from revalidation through Colorado interChange. Health First Colorado providers with a current enrollment (successfully completed revalidation through the Colorado interChange) with the fiscal agent, DXC Technology DXC, do not need to make changes to their enrollment to contract with the RAE(s).
