Please note that the Division of Developmental Disabilities Administrative Office has moved from its Hamilton location to the Department of Human Services building located at  222 South Warren Street in Trenton. This is the physical address only. Mail sent to the Administrative Office should continue to be addressed to PO Box 726, Trenton, NJ 08625-0726.

To Prepare for Fee-for-Service Transition, CCW Individuals/Guardians Encouraged to Begin Researching Support Coordination Agencies

As announced in April, the transition of individuals enrolled on the Community Care Waiver (CCW) into fee-for-service is underway. When a CCW-enrolled individual is identified by the Division to transition to fee-for-service, a notification letter and Support Coordination (SC) Agency Selection Form are sent out to the individual or guardian. The SC Agency Selection Form must be returned to the Division  within two weeks.
 
To reduce the stress of the selection process, the Division suggests CCW-enrolled individuals and/or their guardians begin researching Support Coordination Agencies now. This way, individuals/guardians can be prepared to complete the SC Agency Selection Form when it arrives.
 
The following guides have been developed by The Boggs Center on Developmental Disabilities to assist individuals and families in selecting a Support Coordination Agency:

 
Please note the individual's Support Coordination Agency must serve the county where he/she resides. The Division's list of  Medicaid/DDD approved Support Coordination Agencies, which is updated regularly, identifies the counties served by each agency.
New Acting Chief Executive Officer Appointed at Hunterdon Developmental Center

As announced previously, Hunterdon Developmental Center's CEO, Husam (Sam) Abdallah, is retiring  November 1 . With Sam's departure,  Elisabeth Hutchins will take on the role of Acting CEO at Hunterdon Developmental Center.  Ms. Hutchins has over 30 years of experience in the field of Developmental Disabilities and Mental Health, serving as Assistant Superintendent at Woodbridge Developmental Center and Hunterdon Developmental Center; Quality Assurance Coordinator at Hagedorn Psychiatric Hospital and for the Division of Mental Health and Addiction Services; and Nurse Administrator at Hagedorn Psychiatric Hospital.  In these roles she developed a solid risk management background, developed and implemented policies and procedures, and acquired an extensive knowledge of clinical experience.  Please join us in welcoming Elisabeth to her new role.
