IQVIA™ Institute for Human Data Science Study: U.S. Prescription Opioid Volume Declined 12 Percent in 2017 – Largest Drop in 25 Years
- In 2017, 23.3 billion fewer Morphine Milligram Equivalents (MMEs) were dispensed to patients on a volume basis
- Actual dispensed opioid prescriptions decreased 10.2 percent, while patients receiving high doses (>=90 MMEs/day) declined by 16.1 percent
- Factors driving this opioid prescription decline include major clinical guideline shifts, payer reimbursement controls, intensive medical education efforts by state/specialty medical societies, as well as regulatory and legislative restrictions
- Spending on all U.S. medicines increased 0.6 percent to $324 billion on a net basis due to greater generic volume, larger manufacturer discounts and increasing coupon use
- Medicine spending will grow 2-5 percent after off-invoice discounts and rebates through 2022, to an estimated $380-410 billion on a net basis

Quality data in the 2016 QRUR include the Physician Quality Reporting System submitted by the TIN. Cost data provides the Medicare calculations of cost attributed to each provider in the TIN as it compares to the providers peers across the nation. All TINs nationwide that had at least one eligible professional (EP) bill Medicare under the TIN in 2016-have incurred a bonus or penalty for 2018 Medicare Part B collections. Learn to read your QRUR (very difficult) so you can understand how your care compares to your peers! The QRUR will be used in the MIPS Program Cost Category for 2018!

Opioid deaths prompt Ohio to reimagine classroom lessons, starting with kindergarten
Ohio, a state where 4,329 people
died of drug overdoses in 2016
, a death rate second only to neighboring West Virginia, is taking the fight against the opioid epidemic into the classroom with a new style of drug-abuse-prevention education.
Ohio’s plan, controversial in a state that prizes local control over schools, features lessons that begin in kindergarten. Instead of relying on
scare tactics
about drug use or campaigns that recite facts about drugs’ toll on the body, teachers are encouraged to discuss real-life situations and ways to deal with them and to build the social and emotional skills that experts say can reduce the risk of substance abuse.

Can new payment model boost number of physicians willing to treat opioid patients?
Historically, clinicians have steered clear of becoming certified to treat patients with substance abuse disorders or expanding that patient base because they don't get paid enough to do so or struggle with prior authorization to get reimbursed.
A
new payment model
recently introduced by two leading medical associations aims to increase the number of providers who treat opioid-addicted patients with medication-assisted therapies.
The American Medical Association and the American Society of Addiction Medicine think clinicians should receive an upfront, one-time fee to cover the cost of evaluating, diagnosing and planning treatment for a patient, as well as a month of outpatient medication-assisted treatment. Subsequent monthly payments would be made for continued medication, psychological care and social services.

AHRQ National Guideline Clearinghouse - Website Closing
The AHRQ National Guideline Clearinghouse (NGC,
guideline.gov
) Web site will not be available after July 16, 2018 because federal funding through AHRQ will no longer be available to support the NGC as of that date. AHRQ is receiving expressions of interest from stakeholders interested in carrying on NGC's work. It is not clear at this time, however, when or if NGC (or something like NGC) will be online again. In addition, AHRQ has not yet determined whether, or to what extent, the Agency would have an ongoing role if a stakeholder were to continue to operate the NGC. We will continue to post summaries of new and updated evidence-based clinical practice guidelines until July 2, 2018. For any questions, please contact

Three Conspirators Sentenced in $16.6m Fraudulent Medical Device Scheme
Trio Marketed Laser Devices to Elderly Consumers as Cure-All
Three defendants who marketed and sold light-emitting medical devices as a cure-all to consumers, primarily targeting the elderly, were sentenced on April 20, 2018 by a federal judge in Rapid City, South Dakota, the Department of Justice announced.
Robert “Larry” Lytle, 83, of Rapid City, who was the leader and organizer of the scheme, was sentenced to 12 years in prison, followed by 2 years of supervised release. Ronald D. Weir Jr., 39, of Sioux Falls was sentenced to 24 months in prison, followed by 3 years of supervised release. Irina Kossovskaia, 63, a Canadian resident, was sentenced to 15 months in prison, followed by 2 years of supervised release. U.S. District Judge Roberto A. Lange will issue a final restitution order within 90 days.
Lytle pleaded guilty on January 26, 2018, in the District of South Dakota to one count of conspiracy to introduce misbranded medical devices into interstate commerce with the intent to defraud and mislead, and one count of criminal contempt. As part of his plea agreement, Lytle admitted that beginning in 2005 he entered into an agreement with others to sell medical devices with false and misleading labeling in order to defraud consumers, and that he continued to do so in violation of a federal court order. He also acknowledged that he obtained at least $16,669,015 over the course of the scheme. Lytle made an initial restitution payment of $637,000 and has turned over several thousand dollars’ worth of gold and silver coins to be applied to restitution. Irina Kossovskaia and Ronald D. Weir Jr. previously pleaded guilty for their roles in the fraud. The government agreed to dismiss criminal contempt charges against a fourth individual, Fredretta Eason.
