The Glades PATCH (Planned Approach To Community Health) is a program designed to help the community evaluate, plan and implement health education and disease prevention and intervention programs. PATCH helps promote the health of the community to include social, emotional and physical well-being of its citizens. PATCH is a partnership of concerned citizens and organizations. This partnership provides a forum for identifying health problems affecting the community and mobilizing the community to plan, conduct and evaluate intervention services.
