Dear Provider,

Thank you, to those facilities that completed the REDCap Survey categorizing patient eligibility for transplant.  The data has been given to the National Coordinating Center (NCC) to update your patient denominator data.  For those facilities that have completed the survey, please do so by June 15, 2018. 
 
The transplant journey can be an emotional and anxious time for patients. Navigating through the needs of transplantation can be exhausting process, as well.  This is important to remember as we assist patients with the steps.  Encourage patients to talk with peer mentors, patients whom have completed this journey, and other healthcare team members to discuss fears, challenges, and barriers.  This reminds us all to be caring and supportive during these demanding times.  
 
Resources are provided for the National Kidney Foundation (NKF) Peer Mentoring Program as well as a presentation from the Dialysis Patient Citizens (DPC).  
 
Continue to assist patients through the steps to transplantation.  Please register and participate in the June 19, 2018 NCC Transplant Learning and Action Network (LAN) webinar event.
Project Goals
Participating facilities shall demonstrate a 10% point increase in patients who are on a transplant waitlist by October 2018.
Project Activities
*Complete REDCap Patient Eligibility Survey
*Ensure patient's medical ineligibility is well documented
*Host a lobby day or create an education station for
  patients
