Staten Island PPS and CBC Launch New Program, HEALTHi

SI PPS is partnering with Coordinated Behavioral Care (CBC) on an exciting new project - HEALTHi (Helping, Engaging, and Linking to Health interventions). The program will focus on providing a safety net of resources to  individuals with complex chronic conditions who are also affected by the social determinants of health.

"SI PPS partners are passionate about serving our community members. This new vehicle, HEALTHi, will allow them to bring more resources to those with the most need - early intervention is key," said Dr. Joseph Conte, PPS Executive Director. 

The HEALTHi project will locate and actively engage individuals who have serious behavioral and medical conditions and use wrap-around enhancement funds to address immediate, easily solved social needs, such as food and clothing, in order to secure trust and engagement in care. 

24 Partners Complete Health Literacy Train the Trainer

Staten Island PPS hosted a three-day Health Literacy Train the Trainer with the help of SayAh! to empower new agents to change the way we health-speak. Celina Ramsey, Director  of Health Literacy, Diversity, and Outreach for the PPS, Anna Allen, Co-founder of SayAh! and Marianne Howard-Siewers, Clinical Consultant for the PPS taught the three-day curriculum , consisting  of things like social determinants of health, effective communication, plain language and teach back training, accessible design and more.  Each partner identified a staff person responsible for using the Train the Trainer curriculum to train and empower others in their facility. On the final day of training each partner facilitated a piece of the training, demonstrating how they would use the content to train staff at their sites. 

For Health Coach Margaret Gambale, her most memorable client (PM) was a woman who came to SIMHS with financial hardship.  She had three children and was not from this country. She was also in recovery from alcohol use, and very serious about obtaining employment. She did not have any work experience other than raising children and housework. At the time, employment did not seem feasible. After Health Coach Margaret Gambale spoke with her, they decided together that she needed more help than just referrals to an employment program. Health Coach Margaret Gambale scheduled Ms. PM for an appointment the following week. Together, they worked to create a resume. Once the resume was created, they scheduled an appointment at Workforce 1. Health Coach Margaret Gambale escorted Ms. PM to her first appointment and advocated on her behalf. It took some time, but she did finally gain employment doing housekeeping. Since one of her children is enrolled in SI CARES, Health Coach Margaret Gambale still follows up with Ms. PM and gives her guidance when she needs it. Ms. PM expresses her gratitude for her Health Coach in helping her move forward with her life.



C hristopher Smith, PhD., Director of Managed Care and Adult Services for the New York State Office of Mental Health spoke to the joint Lean Task force of Richmond University Medical Center and Staten Island Mental Health Society. The group of 23 participants from both organizations assembled to use Lean to address the merger/integration issues around improving processes/patient flow.  After a number of classroom sessions where the Lean approach was learned, the group then began to process map their respective work flows to defining bottlenecks and identify opportunities to improve and integrate those processes.
 
On Friday, March 9, 2018 the group moved from their classroom mapping, to conducting an actual "Waste Walk" through the work area. The "Waste Walk" provided a review of the processes mapped in the classroom. Once the "Waste Walk" has concluded the group began to blend their visual observations, classroom mapping and engage staff in dialogue and defining the desired outcomes of improving processes and perhaps job redesign as well. 

Please join us for a presentation on Thursday, April 19th, 2018, with Patricia Bomba, M.D., F.A.C.P., on patient education and empowerment involving the MOLST. MOLST is designed to improve the quality of care people receive at the end of life. MOLST is New York State's Physician Orders for Life-Sustaining Treatment (POLST) Paradigm Program. These programs are based on effective communication of patients wishes, documentation of medical orders on a brightly colored form that is transferable between care settings, and a promise by health care professionals to honor these wishes.

Upon completion of this course, participants should be able to:
  • Understand when a MOLST form should be completed
  • Understand how a MOLST form should be completed
  • Understand how to complete the MOLST form for patients with disabilities or who cannot do it themselves

Staten Island PPS Meets with Healthcare Labor Leaders from Uruguay and Argentina

Senior Director of Workforce Transformation/HR , William Myhre, and health care labor leaders from Argentina & Uruguay met on Thursday, March 22nd, 2018 to discuss DSRIP workforce transformation, sponsored by 1199/TEF. Staten Island PPS and 1199/TEF were joined by the Argentine Federation of Health Care Workers, the National Federation of Health Care Workers Uruguay, and UNICARE. The Argentine Federation of Health Care Workers' Association (FederaciĆ³n de Asociaciones de Trabajadores de la Sanidad Argentina, "FATSA") represents workers in healthcare, home care, pharmacy, pharmaceutical, and emergency services. They represent workers in private and public sectors, as well as for and non-profit healthcare systems in multiple healthcare settings covered by seven national agreements. The National Federation of Health Care Workers of Uruguay (F.U.S.) represents 25,000-30,000 healthcare professionals in Uruguay; most of their representation is in clinical and technical sectors of healthcare including homes care, which is starting to grow in Uruguay. UNICARE is the division of UNI that represents workers in healthcare, home care, early education and childcare. UNICARE has 80+ global unions affiliated to the CARE division.  It was an afternoon of great discussion!

The Department of Health launched their DSRIP Communications Corner, a new initiative intended to provide insights to PPS on what is in the news regarding DSRIP across New York State. Additionally, upcoming events across PPS will be included.

Congratulations to the First Graduating Class of the CHW Accelerated Training Program at the College of Staten Island!

Congrats to the CHW Accelerated Training class at the College of Staten Island! The partnership with CSI, SEIU/1199 TEF, and our employer partners has created a common learning platform to benefit our healthcare partners and the community of Staten Island. Over the past two years we have built a platform which offers skills learning in a very responsive and economically efficient manner. The skills training that we have developed has also put skills training within reach of our Staten Island neighbors, who in the past had no option, such as no MS In Healthcare Administration, or had to travel off the Island to seek skills training.
My Place, An Early Stage Memory Center Grand Opening
On February 12th, the JCC opened a memory center for individuals recently diagnosed with EarlyStage Dementia/Alzheimer's Disease called "My Place: An Early Stage Memory Center." "Early stage" refers to individuals who are diagnosed with Alzheimer's disease or related dementia , and are in the beginning stage of the disease. The early-stage social programs offer meaningful activity and socialization  in a friendly and supportive environment, and they allow participants to remain healthy, active, engaged and independent. 

The hours are Mondays & Wednesdays from 10am-3pm. They are now accepting applications.

Value Based Payment Corner

The SI PPS launched its Value Based Payment Resource Center last month. This resource is hosted on the SI PPS website and supports our overall initiative to educate and prepare the SI PPS Partner Network for the transition from "fee-for-service" to "value based payments," including the SI PPS VBP Education Program.  

The second in-person session of the SI PPS VBP Education Program - VBP Mechanics -  took place on March 20th and covered a range of issues: from basic structures to setting up an arrangement to how MCO's view or manage at-risk agreements. We have added the presentation given by HealthFirst to our VBP Resource Center, along with the supporting document distributed to attendees. We have also added the Greater New York Hospital Association VBP Guide, as we feel this is a great resource for our partners to build on their knowledge of VBP.  

Further content will be posted as our curriculum continues to roll out in 2018.
