Dear Friends,

Although the temperature is dipping below freezing, life at Alameda is warm and cozy. Each of our 6 floors has an ambiance that's distinctly its own, whether its the 3rd floor - home to our Indian Unit replete with cultural décor and cuisine, our Hispanic 5th floor alive with Spanish music and activities, our calm and soothing long-term care units, or our bright and optimistic rehab gym on the ground floor right off our main entrance.

Alameda is a community of care proud to be an integral part of the greater Perth Amboy community. To spread the gift of warmth, we are collecting coats and warm clothing to be distributed by Our Lady of Fatima Church and we welcome your donations! On a clinical note, we have implemented outcome-oriented rehab programs focused on orthopedic, cardiac, pulmonary and neuro recovery suited to meet the specialized needs of our diverse population. And as our patients move forward in their recovery, each has their moment in the spotlight when they receive a memorable VIP red carpet send-off before they return home. In this issue, you will meet some of our Rehab Stars, as well as our Nurse Practitioner and new Resident Council President.

We invite you to come in from the cold to experience the warmth that is unique to Alameda!

We take pride in our Nurse Practitioner who is onsite full-time to assure our residents' medical needs are effectively cared for promptly. NP Sarah Boboli's professional presence is integral to our efforts to reduce hospital readmissions by eliminating needless hospitalizations.

Ms. Boboli assesses the status of all newly admitted residents - reviewing their medications and ordering labs if needed, and works closely with our doctors and nurses to coordinate treatment. After attending daily morning meetings where she is apprised of any patients with immediate concerns, she makes her rounds on each floor. When a resident's temperature is elevated, she looks for the underlying problem - so that the treatment can be effective.
Suffering from shortness of breath and pneumonia, Ms. Lorenza was admitted to Alameda Center from University Hospital. She participated in physical and occupational therapy 5 to 7 days per week to improve her balance and ability to complete functional activities of daily living. Upon her discharge, only 4 weeks later, she was feeling more confident with her balance and no longer required a wheelchair, and was thrilled to be able to complete her daily tasks independently once again.
Mr. Ortiz, a vibrant baby boomer, was admitted to Alameda Center following hospitalization at CareOne LTAC for a stroke. With our supportive therapists at his side, he embarked on his grueling road to recovery with perseverance and grit. Determined to regain his independence, Thomas received physical therapy and occupational therapy 5 to 7 days a week. He benefited from our wide range of state-of-the-art modalities, including the SciFit Stepper - used to strengthen the body while relieving body weight. Within a month and a half, Thomas reached his rehab goals and was ready to return home to his wife.
Michelle Johnson is excited to have recently been nominated as the new Resident Council President at Alameda. A native of Newark NJ, she worked at Walmart in Woodbridge NJ and is the proud mom and grandma of two children and a beautiful grandson named Thomas.

In her new role, Michelle hopes to bring joy and improved quality of life to the residents of Alameda Center. She is an avid knitter and has plans to start a knitting and crochet club in the near future, with residents creating hats and gloves for the needy.
