We are almost half way through this year's QIA performance period. January-September is where we work to meet the QIA goals: Transplant (new patients to the UNOS list) and Home Therapies (new patients starting PD or Home Hemo Dialysis).
May is a good month to review past interventions, and work with existing partners or form new partnerships (Stakeholders, other facilities, providers, and patient groups) to ensure that we are offering patients access to resources and experts in Home Therapies and Transplant.
By the end of May, you should have a firm understanding of how your facility is performing in the QIA. Are you trending to the goal of patients either beginning training for Home Therapies or being listed on the UNOS Transplant Waitlist?

We are working with
Home Dialysis Central (HDC) to help patients, family members and care partners have access to support systems and educational materials to help them make the move to "Home". HDC is a part of the Medical Education Institute (MEI) and they have been building a multi national community of peer support and
Professional resources for years. (They even have an
on-line dialysis machine museum!)
Talk to your patients about the internet, let them know that HDC is available 24/7 for them. If they are feeling isolated, this a space where they can
explore home therapies and connect with others - or just
read their stories.

specialty expertise and knowledge.
