The Institute Sponsored GW Community Health Workers at National Unity Conference

For the past three years the GW Heart & Vascular Institute and GW Hospital have partnered on an innovative program utilizing community health workers (CHWs) to assist outpatients with heart failure. Our GW CHWs, supervised by our nurse practitioner Linda Bostrom, Dr. Gurusher Panjrath, and Dr. Richard Katz, have improved patient quality of life and greatly reduced rehospitalizations. This April, the Institute sponsored our two CHWs, Kimberly Desormeaux and Sawa Kamara, to present at the Unity Conference 2019: The 20th Anniversary of the National Conference for and about Community Health Workers held in Las Vegas, Nevada. The Unity 2019 meeting addressed the role CHWs play in creating access to care, reducing care costs, and promoting healthy and happy communities. Our CHWs presented results of our GW team experience: "Integrating Community Health Workers into Home Based Heart Failure Care."
