Rocking Horse Community Health Center is part of a caring community that encourages, supports and empowers individuals and families to improve their overall health and emotional well being. Rocking Horse Community Health Center three programs in Clark and Madison Counties that United Way of Clark, Champaign and Madison Counties funds. These programs are the CHAMP Care Coordination and Bridges to Healthcare in Clark County, and the Madison County Behavioral Health program in Madison County.
CHAMP stands for Coordinating, Rehabilitation and Maximizing Potential. The CHAMP Care Coordination program provides the seamless delivery of healthcare services for children with special health care needs, regardless of the family’s ability to pay. According to the Data Resource Center for Child and Adolescent Health, one in five children with special healthcare needs have conditions causing financial difficulties for the family. One quarter of these families have cut work hours or stopped working although to care for their child, and those families that still maintain insurance find that coverage is not adequate to meet their needs. The CHAMP program provides a champion for these children for necessary case management and medical advocacy.
The Bridges to Healthcare program provides treatment and management of chronic conditions for under-insured and uninsured residents of Clark County and the surrounding area. In Clark County, a higher percentage of residents are likely to die of heart disease, cancer, stroke and diabetes than in the whole state of Ohio. Socioeconomic statistics indicate that local resident have limited insurance, high rates of poverty and comprised access to regular primary care. This program’s focus is on proper health evaluation, prescription assistance, referrals, health promotion and emphasizes personal accountability to ensure that patients stay on track to better health and costs the community much less than emergency department care.
Low income Madison County residents have had compromised access to healthcare, especially behavioral care. The Madison County Behavioral Health Program’s goal is to increase accessible care to under-insured and uninsured residents for services directly related to pediatric Attention Deficit Hyperactivity Disorder (ADHD) patients and adult substance abuse issues for alcohol and other drugs. This program also increases access to care for behavioral health and psychiatric appointments with a focus on ADHD and substance abuse by utilizing their therapist. The therapist works with other providers to develop an integrated care model that includes completing a diagnostic assessment and providing therapy before referrals are made to the psychiatric provider. This always behavioral interventions to be used before prescribing medications as a first step for a multi-faceted approach to patient need.
