This month we highlight what's coming up for sfCare, open registration for the sfCare Intersectoral Conference on May 24, and provide an sfCare framework handout.

What's coming up for sfCare
In order to assist organizations in effectively applying the sfCare Framework as a holistic template for quality improvement and practice change, we are  developing the following  resources:  
  • Getting Started with Senior Friendly Care implementation package - self-assessment tool, sample sfCare policy, a training module for executive leaders and one for staff (target spring/summer 2018)
  • Senior Friendly 7 toolkit - a toolkit for healthcare providers, patients and caregivers, focusing on 7 priority areas which have a significant individual and additive impact on the independence and well-being of older adults at risk. The toolkit includes resources on: cognition/delirium, mobility/falls, nutrition, polypharmacy, continence, pain, and social engagement (target summer 2018)

We are also advocating for the development of:

  • Standards and indicators - we have submitted a proposal to Health Quality Ontario (HQO) for consideration to develop a suite of senior friendly quality standards and indicators.

Registration open for the sfCare Intersectoral Conference
Senior Friendly Care (sfCare) and the Senior Friendly Seven (SF7): building more connected care for frail seniors
Hundreds of healthcare providers from across Ontario attended the Senior Friendly Hospital ACTION Collaborative Showcase in March 2017 to share their work, collaborate with each other, and hear expert speakers. This year's senior friendly care event will connect healthcare providers across the continuum of care. Space is limited, so we recommend registering early.

When: May 24, 2018, 8:30am-4:30pm
 
Where: Holiday Inn, Yorkdale, Toronto, Ontario
 
Who should attend: any healthcare provider who delivers care with the goal of preserving the health and well-being of frail older adults and those at risk of frailty, including: ED and acute care staff, primary care physicians and teams, home and community care coordinators and personal home support workers, health care planners, and administrators.
 
About: The conference will build upon the sfCare Framework and companion SF7 Toolkit. This event features:
 
Speakers  - series of expert lectures on SF7 topics by:

Collaboration - Structured, facilitated, small-group discussion and co-creation around the toolkits will provide an opportunity for you to share your expertise and provide insights on how to implement SF7 tools in a meaningful way in your practice / your sector / across the continuum of care.
 
Coach's Tip - Make sure everyone is on the same page
Regardless of where your organization is at in its senior friendly care journey, it's important to make sure everyone is on the same page by using the sfCare Framework as the foundation for senior friendly care.
